SIMPSON stated that a neighbor was in a yard cutting grass and he jumped off the
lawn mover and ran away. He stated that she almost hit the man on the lawn mower
and his vehicle. He stated that the trooper was able to hit her car and push her into
the telephone polL SIMPSON stated that he got out of his car and was in the process
of conducting a felony stop. He stated that the trooper ran to the lady's vehicle and
was going to snatch her out. SIMPSON stated that the lady put the car in reverse
and had enough room to back up. He stated that she put it in foJWard again. He
stated that the trooper was running toward the front of her car and SIMPSON yelled
at the trooper to get down and get out of the way because he was in the crossfire.
SIMPSON stated that "she came forward at us and that's when we shot". He stated
that she was trying to come through the hole where they were standing. SIMPSON
stated that he fired approximately 2 shots. SIMPSON stated that the trooper never
fired his weapon and the other shooter was a Glynn County Police Department
Officer.
At that time, Assistant Chief SCOTI TRAUTZ entered the room to monitor the
interview. AGENT SMITH explained again to TRAUTZ and SIMPSON that the case
was a criminal investigation and SIMPSON was not compelled to talk to AGENT
SMITH even though a supervisor had entered the room.
SIMPSON stated that there would be one round missing out of the magazine because
he shot a snake a few weeks prior. SIMPSON stated that he shoots his weapon with
his right hand. SIMPSON went on to explain that he saw both of his shots enter the
windshield. He stated that everything was in slow motion when he pulled the trigger.
He stated that immediately following the shooting, his weapon was taken from him
and he was placed inside a car to sit down. He stated that SGT. CORY SASSER
took his weapon and placed it inside SASSER'S vehicle.
SIMPSON stated that the chase lasted approximately 10 minutes after he became

involved. He stated that there were approximately 10-15 yards between him and the
female driver when he fired the shots. He stated that he and CORY were standing at
CORY'S door when they fired so it could have been 7 yards.
He also stated that his shots were fired in succession without any delay. SIMPSON
stated that he didn't give any commands to the female driver. He stated that he
doesn't remember having to get cover or whether he stood still when he shot. He
stated that everything was in slow motion. SIMPSON stated that he watched his
bullet hit and doesn't remember what he was doing other than that. He stated that he
was shaking so bad that he is surprised her hit her.
He also stated that his vehicle was not equipped with a camera. SIMPSON stated
that he was wearing what he currently had on at the time of the shooting. AGENT
SMITH describes the clothing as a polo shirt with agency badge on the chest area
and a pair of green bdu pants. SIMPSON stated that there were lights and sirens
activated during the pursuit and there was no question that they were the police.
SIMPSON stated that he was using a Glock 30 ACP 45 caliber handgun that was
issued by the department. He stated that he had no prior contact with the female
driver and did not have any prior knowledge of her background or mental state. He
stated that he did not sustain any injuries during the incident and his vehicle was not
damaged. SIMPSON stated that he was not under the influence of any drugs or
alcohol.
SIMPSON stated that he never approached the victim. He stated that as soon as he
shot, he was placed inside a car and his weapon was taken from him. He stated that
he was not directly involved in the chase. He stated that he was attempting to deploy
stop sticks. He also stated that his vehicle is older than some of the others and he
was going to use it to stop her car if the pit didn't work. SIMPSON stated that he
ended up being third in the chase just prior to the shooting. He also made the

comment that it was a miracle that she didn't kill someone.
SIMPSON stated that he has been a police officer for 15 years and had experience
with Brunswick Police Department. SIMPSON stated that he last qualified with his
weapon approximately 3-4 months ago. He stated that he came on duty as the fleet
manager/armorer at 7:00A.M. that morning. He stated that he got a decent night's
sleep the night before and was not fatigued. He advised that he was taking the
medications Kepra, Gym balta and Nexium. He stated that he was not under the
influence of any other drugs or alcohol. SIMPSON stated that he works part-time with
the drug courts on weekends, but he did not work the day before.
SIMPSON stated that he didn't have any background information on the driver or her
mental condition at the time of the call. He stated that his gun should be short 3
rounds and he is fairly certain that he fired 2 times.
SIMPSON stated that he hasn't spoken to anyone else about the event except for
people at the station that asked him about it. He stated that he hasn't been in any
other shootings and does not have any use of force complaints against him.
SIMPSON stated that he didn't have any problems with AGENT SMITH viewing his
personnel file or POST records. He stated that he has never been charged with a
criminal offense.
SIMPSON stated that Officer DIXON also displayed his weapon. He stated that the
man on the lawn mower would have been close enough to witness the shooting
incident.
No other pertinent information was obtained at that time.

a vehicle. TROOPER MALONE added. one of the GCPD officers advised him to take
the lead via a GCPD radio that he had in his car. TROOPER MALONE said he got
behind the vehicle and took the lead in the chase.
According to TROOPER MALONE, the grey vehicle that he was chasing had a flat
tire when he got behind it. TROOPER MALONE said that the vehicle cut through
several parking lots and made several turns before leaving the mall area.
Specifica!ly, TROOPER MALONE recalled seeing a woman run into a building that he
believed was a bank to avoid being hit by the vehicle. TROOPER MALONE stated
that vehicle eventually left the streets surrounding the mall and he followed the
vehicle through several turns, into a nearby subdivision. TROOPER MALONE stated
that the vehicle was all over the road and the driver failed to stop at several stop
signs. Additionally, TROOPER MALONE said the vehicle barely missed striking
several vehicles. TROOPER MALONE added he was unaware of the names of some
of the streets that he and the other vehicle were driving on because he was new to
the area.
According to TROOPER MALONE. as the vehicle was traveling the streets in the
subdivision, the driver drove through a yard and struck a mailbox. The Trooper stated
he noticed a GCPD Officer exit his vehicle to set up spike strips. The trooper added,
the GCPD vehicle was parked on the left side of the road facing him and the GCPD
officer was setting the strips across the right side of the road. TROOPER MALONE
advised that the driver of the vehicle he was chasing appeared to drive directly toward
the GCPD officer, and the ofiicer had to move out of the way quickly. TROOPER
MALONE stated after the vehicle encountered the spike strips, the driver's ability to
control the vehicle lessened.
TROOPER MALONE stated he made the decision to use a pursuit interruption
technique (PIT) to end the chase. According to TROOPER MALONE, he attempted
to PIT the vehicle several times unsuccessfully. TROOPER MALONE advised each

time he attempted the PIT, the driver was able to recover and continue driving down
the road. TROOPER MALONE said he was eventually successful in implementing a
PIT, which resulted in the grey vehicle spinning around to face him and coming to rest
on the right shoulder of the road. TROOPER MALONE stated that he and the GCPD
vehicles then boxed the vehicle in. TROOPER MALONE added he stopped his
vehicle adjacent to the rear passenger side of the grey car.
TROOPER MALONE stated that when the vehicles came to a stop, he exited his
vehicle and drew his sidearm. TROOPER MALONE said he observed the driver
inside the vehicle frantically working the gearshift lever on the steering wheel, and he
holstered his weapon and ran behind her car. According to TROOPER MALONE, the
driver attempted to back into him, but she struck a power pole behind her car instead.
TROOPER MALONE stated that after the grey vehicle struck the power pole, he ran
behind the vehicle to the driver's side. TROOPER MALONE stated he stopped when
he realized the GCPD officers had their weapons pointed in his direction, and he
retreated behind his patrol vehicle. According to TROOPER MALONE, the GCPD
Officers were yelling, but he could not hear what they were saying due to the volume
of the police sirens. TROOPER MALONE stated he had not given any verbal
commands during the encounter. TROOPER MALONE advised that he and the
GCPD officers were all in uniform and had driven marked patrol vehicles.
TROOPER MALONE stated after he returned to the rear of his patrol vehicle, one of
the GCPD officers tried to tell him something. TROOPER MALONE advised he did
not know what the officer was trying to say because he still could not hear due to the
sirens. TROOPER MALONE stated he turned and looked away when the officer tried
speaking to him, thinking there may be another threat in the area. According to
TROOPER MALONE, just as he turned away, he heard several gunshots and he
ducked. TROOPER MALONE did not recall the number of shots he heard.

involved shooting and as a result, he was very nervous following the shots.
TROOPER MALONE stated he did not remember exactly what he had done after he
heard the shots, but he did recall going to his car and turning off the siren.
Additionally, TROOPER MALONE recalled advising the GSP Brunswick Post that
shots had been fired. TROOPER MALONE did not recall if any of the GCPD officers
had rendered aid to the driver of the grey vehicle. TROOPER MALONE stated he felt
that he, the GCPD officers, and the public were in danger immediately following the
chase. TROOPER MALONE added that danger was due the driver's unwillingness to
stop and her erratic driving. Additionally, TROOPER MALONE advised that he had
not fired his side arm during the encounter.
TROOPER MALONE advised that per GSP policy, he used his badge number, 792,
when communicating via the radio. Additionally, TROOPER MALONE advised that
his car is equipped with a laptop computer with a global positioning system (GPS)
installed. Additionally, TROOPER MALONE advised that the GPS in his vehicle
might not have recorded the chase because he had recently removed his laptop from
the car and doing so sometimes affected the GPS unit's reliability. Additionally,
TROOPER MALONE advised that his in-car video camera was running during the
chase and subsequent events.
TROOPER MALONE was unable to provide any additional pertinent information, so
AGENT KELLY concluded the above-described interview at approximately 3:07p.m.
DET. NEU recorded the interview using the GCPD digital video recorder mounted in
the interview room. DET. NEU subsequently gave AGENT KELLY DVD containing a
copy of that video recording, which the agent documented on GBI Receipt for
Property E-525955. AGENT KELLY attached GBI Receipt for Property 525955 to this
summary.

SASSER stated that he ended up as the primary unit in the chase after FARRICK
moved over to the left. He stated that the female driver circled around the mall
parking lot several times making her way toward Embassy Suites and Michael's Deli.
He stated that the Trooper arrived and took the lead in the chase. SASSER stated
that the driver exited the mall parking lot near lhe gymnastics center and was
traveling in the direction of Chapel's Crossing. He stated that he began directing
officers to deploy their stop sticks.
SASSER stated that the female was driving crazy and her right front tire was
completely gone. He stated that they reached Altama and Chapel Crossing and he
10-22 (disregard) the officers behind him running code. He stated that Officer JASON
DIXON went around him to warn oncoming traffic about the hazard. SASSER stated
that the Trooper maintained a safe distance because it was not safe to attempt the pit
maneuver. He stated that the driver took a left into Whispering Pines subdivision.
SASSER stated that the stop sticks were deployed near Shepard Road.
SASSER stated that the Trooper attempted the pit maneuver again, but the chase
was too slow. SASSER stated that TODD SIMPSON joined the chase in his Ford
Expedition. He stated that there were people in their yards and a man on a lawn
mower had to jump off and run away.
SASSER stated that the Trooper initiated the pit maneuver again and tried to push
her car into a telephone pole, but she was too far from the pole. He stated that he
pulled his car up to the front of her vehicle. He doesn't know if he touched her car,
but states that they were hood to hood. SASSER stated that he gave the felony stop
warning over the radio and is not sure if the Trooper could hear that communication.
SASSER informed AGENT SMITH that he got out of this car and drew his weapon.

He stated that he was standing between his driver's door and his car. SASSER stated
that he saw TODD SIMPSON standing to his left. He stated that he saw the Trooper
run to the driver's side of the female's car. SASSER stated that he was giving verbal
commands for the driver to stop the vehicle. He stated that she came forward and
bumped his vehicle and then placed it in reverse and hit the telephone pole.
SASSER stated that he yelled at the Trooper to get out of the way. He stated that
there were 2 people to his left, but he isn't sure if it was TODD SIMPSON or JASON
DIXON or who was standing where. He stated that he could see the driver fumbling
with the shifter and he was yelling at her to stop. SASSER stated that he could see
the car start to move forward. He stated that there was a big enough hole between
the cars that she could fit through. He stated that he was scared for himself and
TODD so he fired his weapon. He stated that he can't recall how many shots were
fired, but guesses 4-5. He stated that his first shot hit her and she jerked back.
SASSER stated tl1at he heard shots to his left, but wasn't sure who else fired. He
stated that the female slumped down in the seat. SASSER stated that he got on the
radio and informed dispatch that shots had been fired. He stated that he took control
of the situation since he was the supervisor on duty. He stated that the female
appeared to be dead when he checked for a pulse and didn't feel one.
SASSER stated that he took TODD'S weapon from him. He stated that TODD
attempted to unload the gun and SASSER told him not to. SASSER stated that he
took TODD'S weapon and his own and placed them on the front seat of his car and
then locked the door. SASSER stated that he let his audio/video run for a while until
CHIEF DOERING instructed him to turn it off. SASSER stated that he sealed off the
area and showed the evidence to the detectives who arrived at the scene. He stated
that he gave CHIEF DOERING and LT. TIN DALE a brief synopsis of the events. He
stated that he was transported to headquarters and left the keys to his car with SGT.

SASSER stated that he had been wearing a Glynn County Police Uniform and had
changed into civilian clothes when he got back to the station. SASSER stated that he
had been employed with the PO since August 2001 and was currently assigned a
Sergeant assigned as a Shift Commander. He stated that he last qualified with his
duty weapon approximately 2 months ago. SASSER stated that he came on duty at
approximately 5:45A.M. that morning. He stated that he got a good night's sleep the
night before after sleeping form 10:00 P.M.-5:30A.M. SASSER stated that he was
not under the influence of drugs or alcohol and was not taking any medication. He
stated that he took some AI eve earlier that morning for back pain.
SASSER stated that he did not know the female driver and still doesn't know her
identity. He stated that he wasn't aware of her past or mental issues at the time of the
chase. SASSER stated that he shot his department issued Glock 21 c 45 Caliber
handgun and uses his right hand to fire it. He stated that his magazine was fully
loaded and he had one round in the chamber. SASSER stated that he was
approximately 10-12 feet from the driver when he fired his shots at her. He stated
that the shots were quick and were fired in succession. He stated that his shots
stopped the threat.
SASSER stated that the female driver wasn't motioning or saying anything to them.
He stated that she was focusing on driving her car. He stated that the guy on the
lawn mower was a witness and approached them after the shooting. SASSER stated
that the video camera in his car was recording everything along with JASON
DIXON'S vehicle.
SASSER informed AGENT SMITH that he had been in a prior police shooting
approximately 5 years ago when he was on bike patrol. He stated that he witnessed
a drug deal and approached a car to investigate. He stated that the car came toward
him so he fired 3 shots. SASSER stated that the suspect pled guilty to aggravated

assault and was sentenced to 10 years. SASSER stated that he had been arrested
when he was in the military, but the charge was dismissed. SASSER didn't comment
on what the charge was. SASSER stated that he had no prior use of force complaints
against him. AGENT SMITH asked SASSER if he had any problems with AGENT
SMITH viewing personnel records or training records. SASSER indicated that he did
not have an issue with AGENT SMITH viewing those records.
No other pertinent information was obtained at that time.

vehicle came to rest. This gouge extended beyond SA ELLIS immediate line of sight
into the distance west of the scene. This gouge appeared consistent with the report
that the vehicle had continued in motion after the tire had separated from the rim.
The positions of all vehicles were marked with pink in color spray paint before SA
ELLIS arrival. Several items of evidence were also marked with cardboard placards
prior to SA ELLIS arrivaL
A Georgia State Patrol Crown Victoria, GA Tag 607 was parked on the northern edge
of the roadway facing a generally southeast direction. Search of the area around the
vehicle revealed nothing of evidentiary value. SA ELLIS also searched the interior of
the vehicle again finding nothing of evidentiary value.
A second Georgia State Patrol Crown Victoria, GA Tag 792 was located southeast of
the previously described vehicle. A piece of red in calor plastic from a vehicle brake
light (Placard 8) was located near the front driver's side tire. This plastic was listed as
Item #1 on GBI Receipt for Property E-527922. The front passenger side quarter
panel/right bumper was located against the rear passenger side quarter panel of the
Buick sedan belonging to SMALL. The front driver's side bumper and push bar of the
Crown Victoria displayed slight body and paint damage.
Search of the vehicle revealed a Glock Model39 semi-auto pistol, serial# GSP1046B
located within an ankle holster wedged between the front seats of the vehicle. A live
round was located within the breech of the weapon, and six (6) live rounds were
located within the magazine, filling it to capacity. There was no indication that this
weapon had been recently fired.
A Glynn County Police Department Chevrolet Impala bearing Georgia Tag GV72736
was located near the southern edge of the roadway southwest of the previously
described vehicle. This vehicle was pointed in a generally eastern direction with the

front push bar against front bumper of the previously mentioned Buick. Two (2) silver
in color Federal brand .45 auto shell casings were located north of the vehicle. The
first (Placard 5) was located northwest of the front driver's side tire. The second
(Placard 4) was located northeast of the rear passenger side tire. These shell
casings were listed as Item #2 and #3 respectively on GBI Receipt for Property E527922.
Two (2) similar shell casings were located on the passenger side near the windshield
(Placard 7) and on the passenger side of the roof of the vehicle near the light bar
(Placard 6). These shell casings were listed as Items #4 and #5 respectively on GBI
Receipt for Property E-527922.
Upon opening the driver's door of this vehicle, SA ELLIS noted two (2) Glock brand
pistols located on the front seat of the vehicle. The magazines were removed from
the weapons and the slides were locked to the rear. A loose Federal brand .45 auto
round was located on the seat by the magazine of each weapon. SA ELLIS collected
a Glock Model 21 C Serial # FSK973 with a Streamlight TLR-1 tactical light. The
magazine for this weapon contained seven (7) live .45 caliber rounds. The magazine
had a thirteen (13) round capacity. SA ELLIS replaced the loose round located
beside this weapon in the previously mentioned magazine. The pistol and the
magazine were then listed as Item #1 and #2 respectively on GBI Receipt for
Property E-522213. The second weapon, a Glock Model30 .45 caliber pistol, Serial
# FPV985 was also located on the seat. The magazine of this weapon contained ten
(10) live rounds. SA ELLIS again replaced the loose live round within the magazine
of the weapon. The magazine also had a thirteen (13) round capacity. This weapon
and the magazine were tl1en listed as Item #3 and #4 on GBI Receipt for Property E522213.
The vehicle also contained a Winchester Model1300 12 Gauge shotgun bearing
Serial #L2547170 and a Remington model870 12 Gauge shotgun, Serial

A black in color Glynn County Police Department Crown Victoria bearing Georgia Tag
GV28596 was located slightly southwest of the previously described Impala. The
vehicle was on the south shoulder of the roadway, pointed in a generally eastern
direction. Search of the vehicle revealed no items of evidentiary value.
A gray in color, Buick Century bearing Georgia Tag AJE 1763 (see attached report)
was located on south side of the roadway. The passenger side wheels of the vehicle
were on the roadway, with driver's side wheels resting on the shoulder of the road.
The vehicle was pointed in a generally western direction. A black in color mailbox and
a wooden telephone pole were located slightly east of the vehicle near the rear
bumper. Landscaped trees and bushes were located south of vehicle.
As previously mentioned the rear passenger side quarter panel was in contact with
the front passenger side bumper of GSP Car 792. Radial skid marks and gouges to
the asphalt consistent with the use of a pit maneuver were readily identified under the
rear of the vehicle. The front bumper was pressed against the push bar of the Glynn
County PD Impala. All four tires of the vehicle were either fiat or missing. The front
driver's window and the rear passenger window on the driver's side of the vehicle
were broken. Broken glass was located on the ground below these windows, along
with quantities of suspected blood and a white in color disposable lighter. Prtor to
moving the vehicle, SA ELLIS noted a defect on the outer rim of the steering wheel at
approximately the 11 o'clock position.

SGT. SASSER told the trooper to assume the lead position in the chase via the radio.
OFFICER DIXON advised he later learned the trooper was JONATHAN MALONE.
OFFICER DIXON stated the police vehicles chased the grey vehicle through several
parking lots, including the Michael's Deli and a dance school near the mall and then
out onto Scranton Road. According to OFFICER DIXON, the chase continued down
Scranton Road to Chapel Crossing Road, where the grey vehicle made a right turn.
OFFICER DIXON stated the grey vehicle was swerving all over the road so he drove
his police vehicle in the left lane to force on-coming traffic to pull onto the shoulder.
OFFICER DIXON stated the grey vehicle turned left into the Waverly Pines
Subdivision and then made the second left heading to the back of the subdivision.
OFFICER DIXON stated OFFICER SASSER advised him via radio to deploy spike
strips to incapacitate the vehicle. According to OFFICER DIXON, he maneuvered in
front of the vehicle and deployed spike strips. OFFICER DIXON stated that as he
was backing up to pull the strips in front of the fleeing vehicle, the grey vehicle
headed straight for him. OFFICER DIXON said he ran behind his patrol vehicle,
pulling the spike strips along with him. OFFICER DIXON said the grey vehicle then
made a left very close to the front of his patrol car, and he observed the driver look
directly at him. OFFICER DIXON stated he yelled for the driver to stop, but she
simply gave him a "yeah, right" look. OFFICER DIXON said he then returned to his
patrol car and rejoined the chase behind OFFICER SIMPSON.
OFFICER DIXON said he observed the vehicle swerve toward OFFICER FARRICK'S
patrol vehicle, which had also entered the neighborhood. Additionally, OFFICER
DIXON said the grey vehicle drove through the front yard of a home in the
neighborhood. According to OFFICER DIXON, TROOPER MALONE attempted to
employ a pursuit interruption technique (PIT), but the vehicle recovered each time.
OFFICER DIXON said the vellic!e then swerved towards a white truck parked on the
shoulder of the road, which OFFICER DIXON knew belonged to MITCH MCDANIEL.

OFFICER DIXON said he also observed MR. MCDANIEL mowing grass in a yard
adjacent to where his truck was parked.
OFFICER DIXON said TROOPER MALONE successfully employed a PIT, and the
fleeing vehicle swerved to the side of the road. OFFICER DIXON explained the
positioning of the patrol vehicles after the chase ended, and AGENT KELLY produced
the attached sketch based on OFFICER DIXON'S explanation. According to
OFFICER DIXON, the PIT had spun the grey vehicle around soil was facing the
pursuing police vehicles, and it came to a rest on the right side of the roadway from
his point of view. OFFICER DIXON added, the GSP vehicle stopped, touching the
grey vehicle along its rear passenger side, SGT. SASSER'S vehicle came to a stop
directly in front of the grey vehicle. OFFICER DIXON said he maneuvered his patrol
car onto the shoulder of the road to the right of SGT. SASSER, and OFFICER
SIMPSON stopped his vehicle in the roadway behind and to the left of SGT.
SASSER'S vehicle. Additionally, OFFICER DIXON stated there were a pole, a street
sign, and shrubs behind and to the right of the grey vehicle from his point of view.
OFFICER DIXON stated all of the Officers exited their patrol vehicles. According to
OFFICER DIXON, he ran behind SGT. SASSER'S vehicle and positioned himself on
the right rear of the GSP vehicle. OFFICER DIXON stated OFFICER SIMPSON
positioned himself between OFFICER DIXON'S vehicle and SGT. SASSER'S vehicle.
OFFICER DIXON later stated he was unsure of the positions of OFFCER SIMPSON
and SGT. SASSER other than they were both to his right. OFFICER DIXON said all
of the officers were giving the driver verbal commands to stop and exit the vehicle.
OFFICER DIXON added, TROOPER MALONE ran behind the grey vehicle and the
driver of the vehicle reversed toward the trooper. OFFICER DIXON said the grey
vehicle struck the pole and came to a stop. OFFICER DIXON advised he was
extremely scared for the trooper's safety at that time. OFFICER DIXON said the
Trooper continued behind the grey vehicle once the trooper realized the pole was
blocking her from going backwards any further. OFFICER DIXON said he observed

the driver of the grey vehicle frantically working the gearshift at that time.
OFFICER DIXON said that TROOPER MALONE then returned to the officer's left
side and he yelled something at the trooper. OFFICER DIXON said that TROOPER
MALONE did not appear to hear him when he yelled. OFFICER DIXON stated the
female driver of the grey vehicle looked directly at him, turned back to the front, and
appeared to let out a sighing breath. OFFICER DIXON stated the wheels on the grey
vehicle turned slightly and the car pulled forward, hitting SGT. SASSER'S vehicle.
OFFICER DIXON stated SGT. SASSER and OFFICER SIMPSON opened fire when
the vehicle began pulling forward. OFFICER DIXON stated he heard several shots
and later learned that eight shots had been fired. According to OFFICER DIXON, the
second or third round that OFFICER SIMPSON fired struck the driver, MS. SMALL, in
her face, and she slumped into the seat.
OFFICER DIXON stated, had MS. SMALL continued to drive the car forward, he
would have fired his weapon too. OFFICER DIXON did not believe MS. SMALL
would have stopped her car for any reason. OFFICER DIXON added he believed
that MS. SMALL would have struck the officers in front of her and kept going.

OFFICER DIXON stated he has been a Georgia Peace Officers Standards and
Training Council (POST) Certified Law Enforcement Officer since 1999. According to
OFFICER DIXON, he had previously worked at Brunswick Police Department and the
Brantley County Sheriffs Office. OFFICER DIXON stated he joined the Glynn County
Police Department in March of 2005. Additionally, OFFICER DIXON stated he was
up to date on all of his training hours. OFFICER DIXON'S last firearms qualification
was in March or April of 2010. OFFICER DIXON stated during his law enforcement
career, he has witnessed two previous deadly force incidents. Specifically, OFFICER
DIXON had witnessed an officer involved shooting on St. Simons Island in late 2009
and an incident in 2001 that he described as similar to the events of June 18, 2010.
OFFICER DIXON stated the video camera in his patrol car was operational.
However, OFFICER DIXON did not believe the video camera had captured a
recording of the shooting because his car was angled away from the grey vehicle.
OFFICER DIXON was unable to provide any additional pertinent information, so
AGENT KELLY concluded the interview at approximately 4:00p.m.
DET. NEU recorded the interview using the GCPD digital video recorder mounted in
the interview room. DET. NEU subsequently gave AGENT KELLY DVD containing a
copy of that video recording, which the agent documented on GBI Receipt for
Property E-525955. AGENT KELLY attached GBI Receipt for Property 525955 to a
previous summary.

CHRISTOPHER STEPAN informed AGENT SMITH that he was present when the
chase and shooting occurred. He stated that he was in the back bedroom watching
television when he heard the sirens. He stated that they sounded like they were
getting closer so he looked out the window to see what was going on.
STEPAN stated that he saw a white car stopped by the telephone pole and could see
the police cars pulled up near it. He stated that the officers got out and drew their
weapons ..He stated that they were screaming at her and were getting louder and
louder. He stated that there were shots fired and he began seeing others officers
show up.

windshield. SA ELLIS completed the attached vehicle sketch at this time. All
measurements are noted on the attached legend. All of the impact sites suggested a
left to right trajectory as viewed from the front of the vehicle. Impact sites 1 and 3
appeared to have entered the windshield at a greater angle than impact sites 4-8. A
projectile was lodged in the windshield at site 2. This projectile was removed and
listed as Item # 1 on GBI Receipt for Property E-527923.
Fine particles of glass powder covered the interior of the vehicle. Search of the front
passenger seat revealed a purple in color purse. The purse contained ~ wallet with a
Georgia Driver's License~ and Social Security Card (IJI.IIIM,I;).for
CAROLINE SYMONDS SMALL. The purse also contained a small
...
containing change, financial transaction cards, a Social Security Ca~rd''li
for MACKENZIE SAVANNAH MCGEHEE, a package of Camel brand cigarettes, a
package of Nicorette gum, a checkbook for CAROLINE SMALL and other assorted
paperwork. The seat also contained an empty Mountain Dew soft drink container and
an apron from Longhorns steakhouse.
The floorboard contained an assortment of empty containers, road maps and other
debris. A single blue in color tablet inscribed with "0 230" and "35" was located on
the floorboard. This tablet was sealed in Block Bag 034608 and listed as Item #1 on
GBI Receipt for Property E-522212 An opened bottle of Taaka brand Vodka and a
metal cookie tin containing a sewing kit were located under the front passenger seat.
The driver's seat contained a red in color cell phone, suspected bloodstains and a
small quantity of glass, a white in color poker chip and several copper projectile
fragments. Another small fragment was located on the door ledge between the driver'
s seat and the driver's door. These fragments were listed as Item #2 on GBI Receipt
tor Property E-527923.

As noted previously, a defect was noted in the steering wheel though during transport
this defect moved from the 11 o'clock position to the 8 o'clock position.
The vertical portion of the seat contained two suspected impact sites. The first
entered the front of the headrest and did not exit. SA ELLIS discovered the projectile
upon cutting open the rear side of the headrest. This projectile was listed as Item #3
on GBI Receipt for Property E-527923.
The second impact site was located in the upper portion of the seat slightly below the
headrest. The projectile entered the front of the seat, impacted a small metal bar
within the seat and deflected slightly upward before exiting the back of the seat.
A third projectile was lodged within the plastic partition separating the driver's door
and the rear passenger door. This projectile was listed as Item #4 on GBI Receipt for
Property E-527923.
A fourth projectile was lodged within the window paneling of the driver's door. This
projectile was listed as Item #5 on GBI Receipt for Property E-527923.
The rear seat of the vehicle contained multiple clothing items, food containers and
household items. Window glass was located on the seat and floor of the vehicle. A
projectile impact was located in the seatback on the driver's side. A projectile was
removed from this site and listed as Item #6 on GBI Receipt for Property E-527923.
A second suspected impact site was located between the window seal/trim of the rear
passenger door on the driver's side. Probing of this area did not reveal and additional
projectile fragments.

On 06/18/2010 at 11:28 AM Glynn County Police Officer Peter Farrick respollded to the area of the Glynn
Place mal! on the report of a female in a gray vehicle possibly heating drugs on her lap. Upon arrival, Ofc. Farrick
made contact with a gray Buick Century bearing a Georgia tag of AJE 1763. The vehicle, being operaled by a
white female later identified as Caroline Smalls fled from Ofc. Farrick. A vehicle pursuit ensued involving other
officers to include Sgt. C Sasser and Officer T_ Simpson. The gray Buick struck a curb whlle exitillg the area of the
Glynn Place mal! flattening a tire. The gray vehicle continued to flee with one tire deflated. Georgia State Patrol
Trooper Malone responded and joined the pursuit. The pursuit led into the Waverly Pines Subdivision where Stop
Sticks were deployed and the three remaining tires were deflated. The gray Buick was stopped by Trooper Malone
using the Pursuit Intervention Technique (PIT) maneuver. Upon stopping, Carolinie Smalls backed her vehicle up
almost striking Trooper Malone who was now on foot. Caroline Smalls !hell pul!ed forward ramm1ng Sgt. Sasser's
patrolcar. Sgt Sasser and Ofc. Simpson fired the weapons at Caroline Smalls striking er an causing her to bring
the gray Buick Century to a stop.

MCGEHEE stated that a plastic surgeon advised him that he could repair the broken
bones in her cheek without any problem and would be able to repair the tear to her
nose. The eye surgeon advised MCGEHEE that the optic nerve in the right eye was
severed and they did not anticipate her vision recovering.
MCGEHEE was informed that it was only a possibility that she would get better, but it
was unlikely. He was informed that a major artery in the back of the brain was injured
and it was too soon to determine if it could be repaired. MCGEHEE informed the
doctors that he did not wish to prolong treatment if the inevitable was going to occur.
He and the doctors discussed removing her from life support.
MCGEHEE stated that SMALL had been an addict for a while and had been through
the drug court program and was in and out of treatment facilities. He stated that she
successfully completed an 18-20 month long term rehabilitation program in
Gainesville, Georgia. He stated that she has had problems with addictions, but he
never knew her to be mean. He stated that SMALL was a member of the AA and NA
communities in Brunswick, Georgia.
MCGEHEE explained that he had custody of SMALL'S oldest daughter. He stated
that SMALL was in constant contact with her daughter. He stated that SMALL
informed him that she had moved into Grace House on Wednesday, June 16,2010.
She told him that she was removed from the house after they discovered a crack pipe
in her belongings. He stated that she was to call him on Friday, June 18, 2010, but
she never did.
MCGEHEE stated that SMALL worked at Longhorn Restaurant in Brunswick. He
stated that he was not familiar with her mental condition and did not know if she had
been diagnosed with any mental disorders. He indicated that her drug of choice had
been heroin. He stated that SMALL'S mother and brother lived in Tallahassee, FL

MCGEHEE stated that he felt the police over responded and couldn't understand why
they shot 8 times and why they struck her in the face.
MCGEHEE informed AGENT SMITH that he would be in contact with her and would

Affidavit having been made before me by lnv. David A Gregory, an officer charged with the duty of enforcing the
Criminal Laws, that he has reason to believe that in Glynn County, Georgia on the following described person,
premises, property, or vehicle; Southeast Georgia Health Systems; 2415 Parkwood Drive, Brunswick, GA 31520
There is now located certain instruments, articles, person(s), or things, namely any and all medical records
pertaining to Caroline Small (02/18/1975) dated 6/18/2010, while a patient at Southeast Georgia Health System.
These records are to include, but not limited to drug I toxicology screening lest results, blood alcohol/ toxicology
results, and or any test results pertaining to blood alcohol I toxicology results identified by another name or any test
result performed to discover the existence of any intoxicant in the system of Caroline Small dated 6/18/2010, while
a patient at Southeast Georgia Health System. Additionally any test result performed to discover the existence of
any infectious disease in the system of Caroline Small dated 6/18/2010, while a patient at Southeast Georgia
Health System. Lastly, a minimum of 12cc of blood, drawn from Caroline Small dated 6/18/2010, which is now
knowingly secured in the Southeast Georgia Health System lab. These items are subject to search and seizure as
tangible evidence of the crime(s) of: aggravated assault with motor vehicle Georgia statute 16-5~21.

On 06/18/2010 at 11:28 AM Glynn County Police Officer Peter Fa~rick responded to the area of the Glynn
Place mall on the report of a female in a gray vehicle possibly heating drugs on her lap. Upon arrival, Ofc. Farrick
made contact with a gray Buick Century bearing a Georgia tag of AJE 1763. The vehicle, being operated by a
white female later identified as Caroline Smalls fled from Ofc. Farrick. A vehicle pursuit ensued involving other
officers to include Sgt. C Sasser and Officer T. Simpson. The gray Buick struck a curb while exiting tile area of the
Glynn Place mall flattening a tire. The gray vehicle continued to flee with one tire deflated. Georgia State Patrol
Trooper Malone responded and joined the pursuit. The pursuit led into the Waverly Pines Subdivision where Stop
Sticks were deployed and the three remaining tires were deflated. The gray Buick was stopped by Trooper Malone
using the Pursuit Intervention Technique (PIT) maneuver. Upon stopping, Carollnie Smalls backed her vehicle up
almost striking Trooper Malone who was now on foot. Caroline Smalls then pulled forward ramming Sgt. Sasser's
patrolcar. Sgt Sasser and Ofc. Simpson fired the weapons at Caroline Smalls strikin her and causing her to bring
