Treatment Plan for 217
Problem area: Standing balance/tolerance
Tx 1: Pt will practice standing tolerance/balance at table with laminated card
game in front of her to also address cognition.
Grade up: Pt will tolerate standing to find 10 matching cards with minimal
verbal cues.
Grade down: OT will cover up all cards that arent being used on board
except for the 10 to minimize distraction, and ask pt to match the correct
cards while standing with moderate verbal cues.
Tx. 2: Pt will practice standing in parallel bars and reaching for cones with L
UE on the right side to increase dynamic standing balance
Grade up: Pt will be standing in parallel bars and reaching for more cones
with L UE on the right side but with R LE placed up on a stepping block to
promote WB in R LE.
Grading down: Pt will be standing in parallel bars and reaching for less
cones with L UE on the left side
Problem area: R hemi and fracture
Tx 1: OT will teach pt how to perform UB dressing using 1-handed techniques
while seated in w/c.
Grade up: Using 1 handed dressing techniques, the pt will don her shirt
without assistance or VC.
Grade down: Using 1 handed dressing techniques, the pt will don her new
shirt with help of OT who will provide hand-over-hand assistance and VC to
complete the dressing activity.
Tx 2: OT will teach pt how to perform LB dressing using reacher and sock aid
while seated in w/c.
Grade up: Using the reacher, the pt will don her pants. OT will assist pt in
standing to help pull pants up. Pt will also don socks using sock aid with no
assistance.
Grade down: OT will help pt thread R LE if necessary before using reacher to
complete LB dressing. OT will assist pt in standing to help pull pants up. Pt
will don socks using sock aid with assistance of placing sock on sock aid and
VC.
