hard to improve the well-being of our region day in and day out. By
providing quality health services to those who need it most, we are
improving the health and happiness of our communities. That not

Hospitals for Health

Parkview Huntington Hospital

Keeping people healthy starts with taking care of their basic


healthcare needs. Parkview Huntington has proudly been supporting the Huntington County Free Health Clinic since 2004.
In 2012, 679 individuals used the free clinic for care at least
once. By providing the funding they need, we can ensure that
our community is receiving adequate, free healthcare.

Parkview LaGrange Hospital

Access to healthcare is one of the leading factors in creating


health and wellness. Because it is a critical need, Parkview
LaGrange makes it a priority to fund medical transportation
in our community through LaGrange County Council on Aging allowing people to get to and from healthcare facilities as
safely and conveniently as possible. LaGrangeCOA.org.

Parkview Noble Hospital

Parkview Noble knows that a mobile community is a healthy


community. Thats why we actively partner with the Cole
Center Family YMCA. By working at local health fairs, creating education opportunities and providing leadership through
board service, were helping our area realize an active future.

Parkview Hospital Randallia and


Parkview Regional Medical Center

Matthew 25 is a free clinic that offers medical, dental and


vision care services for low-income residents that dont have
insurance. The clinic relies on Parkview and our many partners throughout the community to serve over 115 patients
daily. With 18 different specialists regularly available, uninsured patients are able to utilize health services they would
not otherwise have access to. Parkview specifically supports
Matthew 25 by providing laboratory tests, X-rays and referred
emergency room visits, as well as other in-kind services. We
also donate operating dollars to support the clinics ongoing

Parkview Whitley Hospital

Wolf Lake Free Health Clinic serves the health needs of the
uninsured population in Whitley County, free of cost. In
2012, Parkview Whitley supported the clinic through in-kind
lab screenings and medical supplies. Providing this support
allows Wolf Lake Free Health Clinic to continue to offer their
services free to those in need.

Through the generosity of the community, the Parkview


Foundations were able to contribute over $52,800 toward the
Medication Assistance Programs at Parkview hospitals.
Additionally, our hospitals gave over $72,000 to MAP. With
these donations, qualifying patients are able to receive medications they would not otherwise be able to afford.

Plain Church Outreach

As a way to reach the Amish and Mennonite communities,


Parkview LaGrange provides a full-time Plain Church
Coordinator to give them easier access to healthcare. This
includes coordinating a health fair for the Amish parochial
school children that provides them with education and
resources for staying well.

Reaching Out
Project Connect
The Allen County Burmese refugees can have a challenging time communicating to get the care they need.
Project Connect is a partnership between Parkview, St.
Joseph Community Health Foundation and the Allen
County Board of Health. We provide video conferencing
and phone interpretation that has helped nearly 2,000
people receive assistance with scheduling, transportation, insurance and health education.

Francines Friends Mobile Mammography


Parkview Comprehensive Cancer Centers partnership with the Breast Diagnostic Center and Francines
Friends aims to increase the access women throughout
the region have to preventative breast cancer screening. In 2012, more than 4,200 screenings were provided
to women on the mobile coach. Our partnership with
the program covers operational support and follow-up
services to women who need additional testing.
Cancer Prevention
The Parkview Cancer Center staff spent an impressive
439 hours in 2012 sharing the message of cancer prevention to people around our area. Over 9,700 people were
reached at 78 different events, making this a powerful
preventative endeavor.
Home Healths Telehealth
Through donor dollars, the Parkview Foundations were
able to fund the purchase of the landlines required to
run a monitoring program to help low-income Parkview
Home Health patients with congestive heart failure.

Critical Care
With the help of Parkview Foundation donors, $32,000
was raised to fully fund a second critical care transport
unit that serves residents of northeast Indiana and
northwest Ohio with lifesaving care.
Gift of Life
Deciding to be a blood or organ donor is a personal
decision that impacts the lives of people right here in
our community. In 2012, Parkview patients gave the gift
of life to multiple people needing a liver, a pancreas and
kidneys. The generosity of 43 tissue donations helped
provide healing to those who needed it. And, through
the donation of corneas by 91 Parkview patients, 182
people now have the opportunity to see. Each Parkview
hospital also organized blood drives so co-workers and
physicians could easily make a lifesaving gift. In 2012,
blood donations on our campuses resulted in more than
600 units of blood.
LiVe
Whether it was through cooking classes, health screenings, nutrition counseling, festivals or community
partnerships, the Parkview LiVe team shared the
message of easy habits to live by at more than 140
events. Also, the 8 Healthy Habits were distributed to
47,321 public and private school students and teachers
throughout our region.

Parkview Physicians Group Cardiology


PPG Cardiology sponsored the American Heart
Associations BetterU class of 2012. The 12-week Go
Red BetterU program assists women in transforming
their heart health through small, simple choices. We
provided dollars for programming expenses, assisted in
screening applicants, provided two different weekly
education offerings, and also featured a Women and
Heart Disease learning center at the Go Red for
Women Luncheon to celebrate the conclusion of a
successful BetterU.

On a Mission for Health


Creating a healthy outlook includes sharing our talents with other parts of the world in need.
In 2012, the Parkview team went on a total of 11 mission trips, visiting six different countries.
By sharing supplies and medical work totaling over $8,800, people around the globe are
realizing a future full of hope and wellness.
Haiti
Parkview volunteers run a medical clinic in an area of
Haiti that has virtually no healthcare. Most of the people
there only see medical professionals once a year when
our volunteers arrive. This past year, volunteers saw
roughly 1,200 patients in just four days treating infections, stomach ailments malnutrition, diabetes, hypertension and other serious medical conditions.

Stroke Prevention
Getting the word out about stroke awareness is
important to save the lives of our loved ones. Through
efforts like the Strike out Stroke event with the
TinCaps and a monthly support group for those
affected, the Parkview Neuroscience service line successfully reached over 6,780 with our message in 2012.

Dominican Republic
Heading to the Dominican Republic for the 6th
Barnabastask medical mission meant that our team of
38 people were able to do a world of good. Parkview staff
members and several IPFW students, professors and
the Chancellor worked to provide help to an incredible
1,600 patients.
Ecuador
Dr. Alan Yahanda graciously provided his medical services at Dr. Jane Weavers Clinic in San Lorenzo,
Ecuador. As a general surgeon, Dr. Yahanda administered surgical help to men, women and children in need.
Helping the people in Ecuador means giving them the
attention and care required to lead healthier lives.

We understand that your health is affected by many
factors. Through developing a robust economy,
focusing on the big goal of educational attainment,
and ensuring basic needs are fulfilled, we are creating
a more prosperous future for us all.

We are actively working to develop our region, because we


know that healthy economies create healthy communities.

Strong minds can do wonders for an entire region. Without education, growth would simply not be possible.

Parkview Health is the regions largest employer with


over 8,500 co-workers. In 2012, we were listed as the
Largest Employer in Allen County by the Greater Fort
Wayne Business Weekly with 4,710 co-workers working
in the Fort Wayne area.

Supporting Scholarships
An investment in the education of our co-workers and
area youth also means an investment in improving the excellent care for our patients. Through special funds set up
by donors, the Parkview Foundations contributed $17,000
in scholarships to area high school seniors and Parkview
co-workers who were accepted or enrolled in nursing
programs or health-related degree programs in 2012.

Trauma Prevention
Parkviews array of safety programs attempt to reach as many people
as possible with messages that could potentially save their lives, such
as bike safety, Share the Road, Dont Drink & Drive and Think
First. The Dont Text & Drive campaign has been one of our most
successful, with over 137,000 fans on
Facebook.com/DontTextandDriveParkview.
Car Seats
In 2012, the Parkview Foundations provided more than 400 car seats
to families who could not afford them on their own. We are proud to
say that these donations helped ensure a safe start to those
newborns lives.
EMS/Safety Camps
Throughout the region, over 800 children are better equipped
to handle emergency situations through a series of Kids Safety
Camps led by Parkview hospitals. At each safety camp, third
and fourth graders learned important information on how to stay
safe in the water, in case of a fire, at home and on the road, as well
as safety foundations like CPR, first aid and hand hygiene.

Interfaith Mission
Homelessness is an issue in each community that we serve. Recognizing this need, Parkview Whitley Hospital has been a proud supporter of The Lighthouse project since 2001 and has given a total of $125,000 to this
initiative that helps the homeless.

Parkview Field
Getting the community out and about is what Parkview
Field does best. The ballpark, which is home to the Fort
Wayne TinCaps, and Parkview Hospital have
collaborated on numerous health initiatives from fitness
education and blood drives to cancer screenings, bicycle
safety and healthy food choices around the park. Plus,
Stadium Journey Magazine has named Parkview Field as
the top Minor League Baseball experience twice making it one of the most active hot spots in Fort Wayne.

Fort4Fitness and F4F Spring Cycle


Health and fitness is becoming contagious in our community due in large part to the efforts of Fort4Fitness.
Parkview Health is a proud sponsor of the Fall Fitness
events, which bring over 10,000 participants into downtown Fort Wayne to run or walk a 5k, 10k or Half Marathon. In the spring, Parkview Physicians Group sponsors
the F4F Spring Cycle aimed at attracting amateur and
professional cyclists from around the country. Were
proud to be a part of thousands of individuals

SportONE/Parkview Fieldhouse
A 57,000 square-foot multi-sport facility, the fieldhouse
is uniquely designed to get people moving. Its home to
eight volleyball courts, four outdoor sand courts and six
basketball courts a true haven for sports lovers. It also
features a restaurant, training center, weight rooms and
Athletes With Purpose, a sports performance company
that works with area athletes.

Angola Balloons Aloft


This premier, annual event in Steuben County brings
thousands of visitors to the Tri-County Steuben Airport
to see art in the air by way of hot air balloons. Families
and aviation enthusiasts are able to enjoy this community event free of charge due, in large part, to Parkviews
sponsorship. Our support also ensures local nonprofits receive the benefit of such an event. In 2012, Faith
Community Health Clinic and Project Helps Back to
School program each received a gift of $2,000 from the

A bright future starts with an informed present one where goals and objectives
are being set to keep us moving in the right direction.

In 2012, a Community Health Needs Assessment was conducted in order to help


us see just where we should put our focus in the years to come.
Based on this survey, Parkview Health was able to take your feedback into consideration and identify what you believe to be the biggest issues facing your personal
health. By putting our attention on one big goal to create a culture where healthy
living is contagious and convenient, we can take action and become a healthier,
happier community together.

Community Health Improvement (CHI)
Through the Parkview hospitals community benefit
funds, Parkview works with area partners to proactively improve health concerns in our region. Each
Parkview hospital allocates a portion of net
revenue to fund CHI and support local communities. For information on becoming a Health Partner,

Community Nursing
Because we know that the knowledge and skill of
dedicated nurses has a great benefit outside the
walls of our hospitals, we fund the placement of
Parkview nurses strategically throughout the community. They can be found making a difference
in public schools, CANI HeadStart, SCAN, and in

the community addressing health concerns like


diabetes prevention and education, asthma care
navigation and safe sleep environments. For

Parkview Foundations
As a result of generous support from our
community, the Parkview Foundations are able to
invest in the ongoing mission of the hospitals and
programs we support. Annually, the Foundations
contribute an average of $200,000 through donations we receive to help fund Parkview outreach
programming, in addition to patient care resources
and services within the hospitals. Learn

Parkview LiVe
The Parkview LiVe initiative has a vision to create a
culture of healthy living for our area through awareness, education and partnerships. Parkview LiVe
