Synopsis
Parent Initiated. Parent concerned with son's required material list for class. Low income family, cannot
afford all of the items on the list. Empathized with parent, explaining that all students are required to
bring same materials to class in order to ensure success in the classroom. Material list was created so
that if properly used, the materials should last for the entire school term. Assured parent that
communal material will be available if student runs out of something. Offered low cost-ideas for
materials such as: dollar store, HEB, Wal-mart. Suggested to look for community/church programs that

Synopsis
Parent Initiated. Parent contacted via email that student forgot homework assignment in locker and
textbook pages required for reading. Replied at 1040, with the pages required for reading in our
textbook and encouraged parent and student to visit our class website. A PDF of the homework sheet is
located on our "assignments" page that can be downloaded and printed. Took the opportunity to
remind parent and student of classroom policies regarding late homework and its negative impact that it
can have on student's grade. Parent followed up ensuring that it will not happen again and student will
have assignment completed for turn in by Monday (11 JAN) deadline.

Synopsis
Teacher initiated. Contacted parent via on file phone number regarding student consistently falling
asleep in class. Expressed concern that while student is very bright, she is falling behind in class, and
because of her sleeping in class; is often lost in our class discussion. Asked if parent knew of any reason
why student is so tired in class or was aware of her falling asleep in class. Parent was unaware of
situation, but mentioned that student does not begin homework until late (around 2200). Suggested to
parent that she have student start homework earlier so that she can finish earlier and get more sleep at
night. Parent said she will work with student on homework and get her to bed earlier. Follow up sched.
