WRITTEN WARNING

This written warning is issued for your failure to meet the performance expectations of your
position. Specifically, you dont open your slots for at least two weeks and you didnt come to
work last ________ as a result you missed some classes that affected the credibility of our
company. You didnt inform to company of your whereabouts and about your plan of not coming
back to the office for work.
This is your opportunity to correct your unsatisfactory performance and/or behavior. In order for
your performance/behavior to be considered satisfactory, you must achieve and maintain the
following performance standards such as reporting to the office and explaining your reasons as to
why you were not able to come for work and making sure that you will be able to give the
company the necessary information of your absence at least an hour before your scheduled class
shift.
Failure to adhere to the conditions of this written warning, development of new or related
problems, and/or continued unsatisfactory performance will lead to more serious corrective
action up to and including suspension/termination of your employment.
