BIOPSYCHOSOCIAL PART B

Today Friday March 6, 2015 is the second session with client Ray Ronald. We have
already discussed confidentiality, insurance information and an overview of the services our
facility offers. At the last session Ray and the social worker talked about certain issues he was
going through Ray explained that he recently moved back into his parents house since he was
unable to afford rent. He was on the risk of losing his job because of lack of attendance. He also
expressed that he was a heroin addict and has suicidal thoughts. Ray has previously stated that he
was seeking counseling for his addition. Ray expressed has not stayed clean since our last
session and he has stopped attending counseling. The first problem we will be looking at today
will be his drug addiction, which we will work together to come up with a plan to help him get
on the right path to sobriety. The second problem we will be addressing today is Rays thoughts
of suicide. He mentioned that he does have suicidal thoughts and we will be coming up with a
goal to try and help him alter his thoughts from negative feeling to a positive. The client does
have a support system that can help push him along to achieve his goals.

Ray will remain drug free.


A. Ray will call to find a drug rehab facilitation in his area to attend.
1. Ray called the list of drug rehabs provided by the social worker within the metro
Detroit area on March 6th in the social workers office and set up a meeting with
three different locations to get more information. Ray will report on the date and
time at the following locations:
a. The Drugs and Alcohol Prevention Center on Tuesday March 17th at
10:30am. Address: 567 Leslie Lane, Livonia, MI 48293.
b. New Hope Impatient Recovery on Tuesday March 17th at 3:00pm. Address:
2900 Casper St. Detroit, MI 48210.
c. The Drug Rehab of Detroit on Thursday March 12th at 10:00am. Address:
9012 Sunn Dr. Detroit, MI 49209.

2. Ray will report twice a week for a drug test during our sessions on Tuesday and
Saturday afternoons from 1:00pm-2:00pm at the Urban Neighborhood Needs
Building to ensure he is staying clean of any drugs. Address: 224 S Main St.
Detroit, MI 48209.
B. Ray will work on creating new hobbies or activities to do in his spare time.
1. Ray will go to his local YMCA on Michigan Ave. Detroit, MI 3 days out of the
week for a minimum of 30 minutes each day. He will do physical activities to help
boost his energy and become healthier. He will work out at the YMCA on
Monday, Wednesday and Friday between 7:00am-9:00am at 452 Michigan Ave.
Detroit, MI 48209. Ray will keep a log of the hours he went and what he did
while he was there.
2. Ray will attend bi-weekly group therapy sessions hosted at the Urban
Neighborhood Needs on Tuesdays at noon. Address: 224 S Main St. Detroit, MI

Ray will write every day in his journal provided by the social worker about how
he is feeling and why he feels the way he does. Ray will be specific and descriptive

with his journaling. For example he will be answering questions such as:
a. Why do I feel like this?
b. What triggered me to feel this way?
c. What did I do to overcome these feeling? etc.
B. Ray will work on building his support network.
1. Ray will work on strengthening his relationship with his family since he just
moved back in his parents house. He will offer to make dinner twice a week for
the family, since he enjoys to cook. He will ask his family to help in preparing the

food as family bonding time. Ray will report at each session what food he made
and who help him create the dish.
2. Ray will go out with his co-workers once a month to have a good time and laugh.
He will suggest to them that they should have a get together on the Third Saturday
of every the month. Ray will report on it at the following Tuesday session. The
first outing will be March 21, 2015.
