It is evident that there were inconsistencies, poor record keeping, poor
administration, no system to correlate with contract payment from Management
‘Accounts, no monitoring of the project to verify change orders and so much more
that is left for discussion because of the lack of process that was not
interconnected to the business and process mapping goals for the Fox Hill
Treatment Plant for contract administration. As a result many problems occurred
and are identified below to create the framework and reference point for the
analysis.
Problem Statement
The lack of properly maintained project records to show project steps as it relates
to the original budget and correlates with: specifications, standards, pricing
conflicts in terms of labor and cost, plant installation and inspections, planning,
design, change orders, bidding, post construction as it relates to the Fox Hill
Treatment Plant to protect Water & Sewerage’s interests
Causal Identification As It Relates To Problem
In August 2004, the Fox Hill Treatment Plant was not working properly, sewage
treatment was suspended, flows were diverted into deep well disposal because
the number two tank was ruptured.
At the planning phase SABL was in negotiations with WSC executives and from
documented records it is evident that there was no formal process in place to
deal with the prequalification of bidders exercise. The contract was awarded to
SABL Bahamas and Mr. Glenn Archer assumed the responsibility to protect
WSC interest as the Senior Manager of Sewerage Operations, and Mr.
Christopher Sherman, Deputy General Manager — Engineering_and Planning
which includes Project Management, as Project Manager and Division Head
which clearly is his role within WSC organizational structure and the Fox Hill
Treatment Plant Project by virtue of his title.
In order to address possible root causes to the problem a review of the
documentation was read and based on the facts and questions asked by ICC,
