APPLICATION FOR MPH(H) 2016-17: INFORMATION SHEET

APPLICATION GUIDE
1. Application form is to be filled online only.
2. Upload your recent passport size colour photograph.
3. Attested copies of following certificates should be submitted along with the
Application:a. Pass certificate/Degree certificate
b. Marks cards of all university examinations passed (First to final year of
the course)
c. Attempt Certificate issued by the Principal
d. Certificate regarding recognition of the college by the respective councils /
regulatory body
e. Completion of rotating internship certificate form a recognized college
where applicable
f. Registration certificate by the respective state council
g. Proof of SC/ST/OBC/HBK /in-service as the case may be
4. Application fee is Rs.500/-. The fee has remitted through online payment gateway at
www.rguhs.ac.in. Select >Student Fees, > Student Fees
5. Print out of online filled application along with attested copies of documents should
be submitted either in person or by post so as to reach the office of Registrar on or
before 30th April 2016, 5:00 PM

1. Venue : Dhanvantri Hall, Rajiv Gandhi University of Health Sciences,
4th T Block, Jayanagar, Bangalore 560 041
2. Day, Date and Time : Saturday, 21stMay, 2016 at 3 PM

Question paper will consist of 50 objective questions.


Each question carries one mark. There is no negative marking.
The examination will be held for duration of one hour.
