Section1300.400ApplicationforLicensure

a)Anapplicantforlicensureasanadvancedpracticenurseshallfileanapplicationonforms
providedbytheDivision.Theapplicationshallinclude:

1)CurrentIllinoisregisteredprofessionalnurselicensenumber.

2)Proofofcurrentnationalcertification,whichincludescompletionofanexamination,
fromoneofthefollowing:

A)NurseMidwifecertificationfrom:

i)theAmericanCollegeofNurseMidwives(ACNM)or

ii)theAmericanMidwiferyCertificationBoard.

B)NursePractitionercertificationfrom:

i)AmericanAcademyofNursePractitionersCertificationProgramasa
NursePractitioner

ii)AmericanNursesCredentialingCenterasaNursePractitioner

iii)ThePediatricNurseCertificationBoardasaNursePractitioner

iv)TheNationalCertificationCorporationfortheObstetric,Gynecologicand
NeonatalNursingSpecialtiesasaNursePractitioneror

v)TheCertificationBoardforUrologicNursesandAssociatesasaUrologic
NursePractitioner.

C)RegisteredNurseAnesthetistcertificationfrom:

i)CouncilonCertificationoftheAmericanAssociationofNurse
Anesthetistsor

ii)CouncilonRecertificationoftheAmericanAssociationofNurse
Anesthetists.

D)ClinicalNurseSpecialistcertificationfrom:

i)AmericanNursesCredentialingCenter(ANCC)asaClinicalNurse

3)TheBoard,inadditiontothecertificationslistedinsubsection(a)(2),mayreviewand
makearecommendationtotheDivisiontoacceptacertificationifthecertifyingbody
meetsthefollowingrequirements(certificationsarelistedinAppendixA):

A)isnationalinthescopeofcredentialing

B)hasnorequirementforanapplicanttobeamemberofanyorganization

C)hasanexaminationthatrepresentsaspecialtypracticecategory

D)hasanexaminationthatevaluatesknowledge,skillsandabilitiesessentialforthe
deliveryofsafeandeffectivespecialtynursingcare

E)hasanexaminationwhosecontentanddistributionarespecifiedinatestplan

F)hasexaminationitemsreviewedforcontentvalidity,culturalsensitivityand
correctscoring,usinganestablishedmechanism,bothbeforeuseandperiodically

G)hasanexaminationevaluatedforpsychometricperformance

H)hasapassingstandardestablishedusingacceptablepsychometricmethodsandis
reevaluatedperiodically

I)hasexaminationsecuritymaintainedthroughestablishedprocedures

J)issuesacertificationbaseduponpassingtheexamination

K)hasmechanismsinplaceforcommunicationtoboardsofnursingfortimely
verificationofanindividual'scertificationstatus,changesincertificationstatus
andchangesinthecertificationprogram,includingqualifications,testplanand
scopeofpracticeand

L)hasanevaluationprocesstoprovidequalityassuranceinitscertificationprogram.

4)Proofofsuccessfulcompletionofagraduatedegreeappropriatefornationalcertification
intheclinicaladvancedpracticenursingspecialtyoragraduatedegreeorpostmaster's
certificatefromagraduatelevelprograminaclinicaladvancedpracticenursingspecialty.

5)Anapplicantseekinglicensureinmorethanoneadvancedpracticenursingcategoryshall
havemettherequirementsforatleastoneadvancedpracticenursingspecialtyand

A)Submitproofofpossessionofanadditionalgraduateeducationthatresultsina
certificateforanotherclinicalAPNcategoryandthatmeetstherequirementsfor
thenationalcertificationfromtheappropriatenursingspecialtyand

B)Submitproofofacurrent,nationalcertificationfromtheappropriatecertifying
bodyforthatadditionaladvancedpracticenursingcategory.

6)VerificationoflicensureasanAPNfromthestateinwhichanapplicantwasoriginally
licensed,currentstateoflicensureandanyotherstateinwhichtheapplicanthasbeen
activelypracticingasanAPNwithinthelast5years,ifapplicable,stating:

A)Thetimeduringwhichtheapplicantwaslicensedinthatstate,includingthedate
oftheoriginalissuanceofthelicenseand

B)Whetherthefileontheapplicantcontainsanyrecordofdisciplinaryactionstaken
orpending.

7)ThefeerequiredinSection1300.30(a)(4).

b)AnapplicantforlicensureasanAPNmayapplytotheDivisionforatemporarypermit,onforms
providedbytheDivision,topracticeasanAPNpriortotheissuanceoftheAPNlicense.

1)ApplicationRequirements

A)Theapplicationshallincludeacompleted,signedapplicationforlicensure,asset
forthinsubsection(a).

B)Theapplicationshallincludedocumentationfromanapprovedcertifyingbodyset
forthinsubsection(a)(2)indicatingthedatetheapplicantisscheduledtositfor
theexamination.Uponsuccessfulcompletionoftheexamination,proofof
certificationshallbesubmittedtotheDivisionfromthecertifyingbody.

C)AnAPNwhowillbepracticinginahospitalorambulatorysurgicaltreatment
centerinaccordancewith210ILCS5/6.5shallnotberequiredtohave
prescriptiveauthorityorawrittencollaborativeagreementpursuanttotheActand
thisPart.

D)AnAPNapplicantwhowillbepracticingoutsideofahospitalorambulatory
surgicaltreatmentcentershallprovideacertifyingstatementindicatingthatthe
APNapplicanthasenteredintoacollaborativeagreementasrequiredbySection
6535oftheAct.

E)TheapplicantshallincludetheprocessingfeesetforthinSection1300.30(a)(4).

2)PracticePendingLicensure

A)TheDivisionwillprovidealettertoeachapplicantindicatingtheabilityto
practicependinglicensure.

B)Practicependinglicensureshallbeterminatedupon:

i)theissuanceofapermanentlicense

ii)failuretocompletetheapplicationprocesswithin6monthsfromthedate

ofapplication

iii)afindingbytheDivisionthattheapplicanthasviolatedoneormoreofthe
groundsfordisciplinesetforthinSection705oftheAct

iv)afindingbytheDivisionthat,withinthelast5years,theapplicanthashad
alicenseorpermitrelatedtothepracticeofadvancedpracticenursing
revoked,suspendedorplacedonprobationbyanotherjurisdiction,ifat
leastoneofthegroundsissubstantiallyequivalenttogroundsinIllinoisor

v)afindingbytheDivisionthattheapplicantdoesnotmeetthelicensure
requirementssetforthinthisSection.

C)TheDivisionshallnotifytheapplicantinwritingoftheterminationandshall
notifytheapplicantbycertifiedorregisteredmailoftheintenttodenylicensure.

c)Whentheaccuracyofanysubmitteddocumentationortherelevanceorsufficiencyofthecourse
workorexperienceisquestionedbytheDivisionortheBoardbecauseoflackofinformation,
discrepanciesorconflictsininformationgiven,oraneedforclarification,theapplicantseeking
licensureshallberequestedto:

1)Provideinformationasmaybenecessaryand/or

2)AppearforaninterviewbeforetheBoardtoexplaintherelevanceorsufficiency,clarify
information,orclearupanydiscrepanciesorconflictsininformation.

d)AnAPNlicensemaybeissuedwhentheapplicantmeetstherequirementssetforthinthis
Section.
