Informed Consent Form for UP Womens Taekwondo
This informed consent form is for the UP Womens taekwondo athletes invited to
participate in a research study titled Effects of Various Heights in Depth Jump Leg
Kinetics and Kinematics of female College Taekwondo Athletes.
This Informed Consent Form has two parts:

Information sheet (to share information about the study with you)
Certificate of Consent (for signatures if you choose to participate)

You will be given a copy of the full Informed Consent Form


Part I: Information Sheet
Introduction
We are Vinelle Chua, Justin Dizon, John Enriquez, and Jao Nasis, Bachelor in Sports
Science students of UP DIliman. We are conducting a research on the effects of
various heights in depth jump leg kinetics and kinematics of female college
taekwondo athletes as a major requirement for our SS 152 Biomechanics II class.
We are going to give you information and invite you to participate in our study. If
you still have questions after reading the form, please do not hesitate to ask any of
the researchers for clarifications.
Purpose of the research
The purpose of this research is to determine effects of different depth jump heights
on kinematic and kinetic variables.
Type of Research Intervention
This research will involve your participation as test subjects performing several
depth jumping tasks using different box heights.
Participant Selection
You were invited to take part in this research because your experience and
participation in the field of taekwondo are critical to the reliability and validity of this
study.
Voluntary Participation
Your participation in this study is entirely voluntary. It is your choice whether to
participate or not. If ever you agree, you have the freedom to withdraw participation
later in the study if you change your mind.

Procedures
Your participation will enable us to gather data about depth jump kinematics and
kinetics in varying depth jump heights. If you accept, you will be asked to perform
several jumping tasks which will be recorded with a video camera and contact
platform.

The test comprises of 3 jump heights: 30cm, 50cm, and 80cm, with a 30 sec
rest in between trials and 2 min rest between heights
Duration
Your participation will only be a single day and last for approximately half an hour.
Risks
You will be performing a plyometric exercise to be recorded by a video camera. A
carefully designed warmup routine will prepare you for the activity. However,
researchers will not be held responsible for any unfortunate event that may occur
during testing.
Benefits
There will be no direct and immediate benefit to you, but your participation will
contribute to expanding the knowledge of the science and advancement of your
field (taekwondo) and other related fields.
Confidentiality
We will not be sharing any information about you to anyone outside of the research
team. The information including the video footage that will be gathered will be kept
private. Information relevant to the research will have a letter instead of your name.
Only the researchers will know your letter and that information will be kept private.
If ever we see fit that the video or a screenshot of you from the video shall be used
in our paper, we will contact you for permission.
Sharing the Results
The knowledge that we will get from this research will be shared. Each participant
will receive a copy upon request.
Right to Refuse or Withdraw
You do not have to take part in this research if you do not wish to do so. You may
stop participating at any time that you wish without any consequence. You may see
the footage at the end of the session if you wish.
