PETITION
COMES NOW, the undersigned Petitioner unto this Honorable Sharia Circuit
Court, most respectfully alleges:
1. That the Petitioner is of legal age, Filipino and a resident of Block 5, Lot 13,
Phase I, General Santos City;
2. That he is employed as Police Officer III of the Philippine National Police (PNP)
and presently assigned at Headquarters Regional Services Group (HRSG), Police
Regional Office 12 at Barangay Tambler, General Santos City;
3. That his estranged wife, SIRIKIT DANI GAMPONG, is presently employed as
Rural Health Midwife at Acharon Health Center in General Santos City;
4. That they were married in accordance with Islamic Rites on March 22, 1989 in
Dalumangcob, Sultan Kudarat, Maguindanao;
5. That they were also married before Hon. Samuel V. Ferenal, Acting Municipal
Trial Judge of Dalumangcob, Sultan Kudarat, Maguindanao. Copy of the Marriage
Certificate is hereto attached and marked as ANNEX A;
6. That due to irreconcilable differences between the petitioner and his wife, the
said marriage bond was severed by virtue of Talaq, a divorce effected by the petitioner on
March 14, 2006 in accordance with Article 46 of Presidential Decree No. 1083, otherwise
known as Code of Muslim Personal Laws, which was signed into law on February 4,
1977;
7. That they have remained separated and apart from each other since March 14,
2006;
8. That copies of the agreement made by the parties before the Council of Elders
and Maguindanao Provincial Committee are hereto attached as ANNEX B and C,
respectively, and made as an integral part hereof;
9. That the Petitioner agreed to support his children by Sirikit D. Gampong and
currently observing and complying with his obligation to this effect;
10. That the Petitioner seeks for judicial recognition of the DIVORCE effected by
him thru talaq;
11. That the Petitioner wants to effect changes on his civil status written in his
personal records with the National Police Commission (NAPOLCOM), Philippine
National Police (PNP), Local Civil Registrar of Sultan Kudarat, Maguindanao, and
National Statistics Office (NSO);

12. That the Petitioner prays that a Decree of Divorce be issued.

WHEREFORE, IN VIEW OF THE FOREGOING, the undersigned Petitioner

respectfully prays for the following:
1. That the DIVORCE effected by the petitioner be given JUDICIAL
RECOGNITION;
2. That a DECREE OF DIVORCE be issued; and
3. That an ORDER be issued commanding the National Police Commission
(NAPOLCOM), Philippine National Police (PNP), Local Civil Registrar of
Sultan Kudarat, Maguindanao and National Statistics Office (NSO) to effect
changes on the civil status of the herein Petitioner from MARRIED to
DIVORCED;

COMES NOW, the undersigned petitioner unto this Honorable Registrar of
Deeds of Sultan Kudarat, most respectfully alleges:
1.
That the petitioners is of legal age and a resident of Tacurong City,;
2. That she is one of the co-owners of parcel of land by virtue of Deed of
Extrajudicial Settlement executed before the undersigned Notary Public, the
said document hereto attached as ANNEX A;
3. That the parcel of land is covered by Transfer Certificate of Title No. T-41492
known as Lot 192-D-17, Psd-12-022737, situated at Brgy. Tina, Tacurong
City;
3. That a subdivision of the said land had been caused into different lots as
appearing on the subdivision plan duly approved by the DENR, Office of the ARED for
Operations, Region XII, Koronadal City on Sept. 26, 2002, hereto attached as ANNEX
B;
WHEREFORE, IN VIEW OF THE FOREGOING, it is most respectfully
prayed that said lot under Transfer Certificate of Title No. T-41492 be segregated in
accordance with the following:

legal age and a resident of Tacurong City, and ARNOLD T. CABAGUING, Filipino, of
