NARRRATIVE GENOGRAM

FAMILY HEALTH STATUS

According to the patient he has no idea about his grandparents in his mother side
because the time he was born both pass away, while his father side his grandmother also
pass away and only his grandfather who was still alive but when the patient reaches high
school, age 14 years old his grandfather died due to the complications of Diabetes
Mellitus. He also indicate that he already forget the health history and the reason why his
ante and uncle pass away.

BIOLOGICAL PARENTS AND CHILDREN

A and B got married and has 7 children. He states that he has no information
about his siblings because he grows up with his uncle in Davao City. And he only knows
that the second sibling has asthma. And treated with herbal medicine such as Lagundi
leaves seldom, his father boiled lizard because of their superstitious belief.

PAST HEALTH STATUS

 The patient claimed that since he exercises regularly and doing simple works and
farming. He catches minor illness like cough, fever, colds and simple wounds that are not
easily healed. He smokes since his adolescence stage age 17 years old and can consumed
one pack a day and alcoholic beverages and coconut wine known as “tuba”. Patient X
was not given passive immunity because their family lacked of knowledge about
immunization and no one recommended them about immunization because they were
born in their house or “hilot”.

During the adolescent time, patient X got hospitalized due to vehicle accident.
Patient X aged 79 was being diagnose with Diabetes Melllitus type 2 since 2009 with
irregular medication then stopped without follow up check up.

PRESENT HEALTH STATUS

A month prior to admission, patient X had secretion on right lower extremity due
to itchiness with created deep wound, no consultation and no medication taken until such
time it became infected, swelled and redness.

Prior to admission, patient X was diagnosed with Cellulitis in right lower

extremity and Diabetes Melllitus type 2.
