GUIDELINES TO FILL FORM PF TRANSFER FORM13

1.column1-here mention your name as mention in form 2 nomination form when you
joined the company
2.column2- please mention here your fathers name as given in your previous
company.
In case of married women in case you have given your husbands name earlier you
can
state the same.However if your martial status has changed since then please
continue with
earlier details-change of name now will cause rejection of your application.
3.column3-here please mention your previous company full particulars with
complete
postal address
4.column 4 here mention your pf number with correct prefix/company code/followed
by
your number assigned by your previous company
5.column 5-here give the full address of the rpfc who holds your pension fund.
6column 6-here mention your pension fund no as given by your previous company
first prefix/then company code no/followed by the no allotted for you.
7.column7-here enter the date of leaving correctly
8.column 8-here give the correct date of joining Oracle
Then sign the application manually.
Thereafter down under please mention Oracle address which should be oracle –OTP-
Bannergatta road address only-no matter in which location you are
employed/serving.
Then the current pf no-in your pay slip at Oracle you will find the pf no which
would
read as KN/42357/followed by your no.
Then in the EPS account no- mention once again the same no

MOST IMPORTANT
It is our request to you all to approach your previous employer and request them
to
forward form 3a to you.If you enclose the same along with your transfer
application the
process can be speeded up and the transfer will be effected in ¾ months time.
Otherwise it will take a minimum of six months time for the transfer.
The time limit indicated is only indicative and the member should not quote this
since the
transfer has got to be received from another region over which we will not have
any
control.
Please note that you will not receive any statement of this transfer
individually and the
transfer amount will be received directly by RPFC Banglore .However this
transferred
amount will be reflected in the annual statement which the RPFC will give at the
end of
the financial year.
