Résumé. – Depuis son introduction par Norman Holter en 1967, l’enregistrement ambulatoire de longue
durée de l’électrocardiogramme (ECG) a démontré son intérêt inestimable pour le diagnostic et la prise en
charge des troubles du rythme. Les principales indications de l’enregistrement Holter ECG chez l’enfant
concernent : l’évaluation de symptômes intermittents possiblement d’origine rythmique ; la surveillance du
rythme cardiaque dans certaines cardiopathies ou affections pouvant se compliquer de troubles du rythme ; la
surveillance des traitements antiarythmiques.
L’intérêt du test d’inclinaison pour rattacher certaines syncopes de l’enfant à une origine vasovagale est
maintenant couramment admis. Il est indiqué chez les patients ayant des syncopes récidivantes ou atypiques
et ne doit toutefois pas être systématique chez les enfants présentant des syncopes vasovagales typiques dont
l’examen clinique et l’ECG sont normaux.

1 Tracé Holter obtenu chez un enfant de 10 ans adressé pour syncopes et précordialgies d’effort. À l’occasion d’une émotion (jet de pétard), survenue d’une douleur précordiale
contemporaine d’une tachycardie ventriculaire bidirectionnelle permettant d’évoquer le diagnostic de tachycardie ventriculaire catécholergique.

3 Tracé Holter d’un patient opéré 10 ans auparavant d’une intervention de Fontan
qui se plaignait d’accès de palpitations. Lors d’un tel accès, survenue d’une salve de ta-
chycardie atriale de neuf complexes. Les trois premiers complexes et le dernier sont si-
nusaux, ils mettent en évidence l’importante hypertrophie auriculaire droite habituelle
après ce type de chirurgie.

6 Tracé Holter d’un patient de 10 ans ayant une tachycardie permanente diurne et nocturne avec un rythme nycthéméral plat. Pendant le sommeil survenue d’un épisode de bloc
