I release, waive, discharge and relinquish the Municipal Government of Bustos, Bulacan and their officers, employees

members and agents from any liability, loss damage, claim demand or cause of action against them attributable to the
participation in this competition, whether the same shall arise by their negligence or otherwise. I agree to indemnify and
not hold the Municipal Government of Bustos, Bulacan responsible for any incident, accident, injury or illness that may
arise out of participation with this event. I agree that photographs, pictures, slides, movies or videos of the choir may be
taken in connection with our participation with or without compensation from the Municipal Government of Bustos,
Bulacan, and consent to the use of photographs, pictures slide, movies or videos for any legal purpose. I warrant that
the members of the choir are in good health and have no physical condition that would prevent them from participation.
