an up-to-date record of your child’s immunizations

any pertinent medical or psychological records that may apply

to your child

all disciplinary records concerning your child

any prior Individual Education Plan (IEP), Service Plan (SP),

504 Plan, or any similar accommodation plan for your child

emergency contact information

previous and current grades, including standardized test

scores (or the full address of the school from which these
documents may be obtained)

your nonrefundable application fee of $65.00

your nonrefundable test fee of $40.00 ( this applies to 1st

through 8th grades)

beneficial in determining the admission and/or placement of my child.
