    1) A work stop occurred at the TCOT tank T-446 due to a lack of proper permits and safety documents.     HOTWORK PERMIT was not issued Fire hydrant hoses and fittings

    COLD WORK cannot be carried out:


    1) Emergency
    overflow roof to remove vapor seal
    JHA and JobCard was not attached
    2 Qty: 19 nos
    to PTW
    2) To
    service and cleaning rim vent (6")

    Recovery plan

    1) To purchase immediately fire hydrant and fittings


    (Purchase date: 17/02/2011)

    2) JSA and JobCard was issued and accepted by all work supervisor.
    (Date: 17/02/2011)

    Lesson Learnt

    1) To avoid communication breakdown between BULER project team and site


    workers.

    2) To ready all related document prior to start any work.

    3) To regularly educate all supervisors and workers about the work process flow,
    (if applicable e.g EIS, SOE, MCS, JMS, drawings, work schedule, manpower and
    equipment loading, JSA, repair procedure, consumable, MOS, parts, material and etc
