AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, REBECCA ESTRADA, a resident of Nagrebcan, Bauang, La Union and

ELENO PADRE, a resident of Ora Centro, Bantay, Ilocos Sur, both of legal age,
after having been duly sworn to in accordance with law hereby depose and state:

1. that we personally know CIPRIANO PALACPAC who married MARISSA

ABUBO on 23 September, 1974 at Bauang, La Union and who died on 05 July
2010;

2. that CIPRIANO PALACPAC is the son of the late Sps. Pedro Palacpac and
Juana Patron and who was born on 16 September 1946 at Bantay, Ilocos
Sur;

3. that CIPRIANO PALACPAC and MARISSA ABUBO have one (1) child by the
name of Marilyn Abubo Palacpac;

4. that CIPRIANO PALACPAC is the same person as ROGELIO PALACPAC who

worked at the Provincial Jail in Vigan, Ilocos Sur and a member of the PAG-
IBIG Fund;

5. that we hereby attest that CIPRIANO PALACPAC and ROGELIO PALACPAC

refer to one and the same person.

I, KENNEDY CHAN, of legal age, married, Filipino, and a resident of

Ilocanos Norte, City of San Fernando, La Union, after having been duly sworn to in
accordance with law hereby depose and state:

6. that I am using KENNEDY M. CHAN as my name in all my official

transactions as it was my registered name as appearing in my Certificate of
Live Birth;

7. that in my PhilHealth account, I used my name KENNEDY CHAN;

8. that however, in the Official Receipt of my cash bond in Criminal Case No.
7438, I used my alias, KENNETH CHAN;

9. that I vouch that the herein affiant and the persons abovementioned with
the names KENNETH CHAN and KENNEDY CHAN, refer to one and the same
person;

10. that I am executing this affidavit to attest to the truth of the foregoing
narration of facts and for all legal intents and purposes it may serve.

AFFIDAVIT OF DISCREPANCY

I, LARRY CRISPIN MONTAÑEZ y Barnachea, of legal age, married,

Filipino, and a resident of Parian Este, Bauang, La Union, after having been duly
sworn to in accordance with law hereby depose and state that:

11. I was using LARRY CRISPIN as my first name in all my official transactions as
it was my registered first name appearing in my Certificate of Live Birth on
file at the Local Civil Registrar, Bauang, La Union;

12. However, in my Marriage Certificate my first name was inadvertently placed

LARRY and not LARRY CRISPIN;

13. I vouch that the herein affiant and the persons abovementioned with first
names LARRY and LARRY CRISPIN, refer to one and the same person;

14. I am executing this affidavit to attest to the truth of the foregoing narration
of facts and for all legal intents and purposes it may serve.
