Affidavit of Truth

The undersigned, being first duly sworn, deposes and says:

I, FirstName LastName, living woman (henceforth Affiant), Authorized Representative of

the person known as FRISTNAME LASTNAME, do swear and affirm that, as a physical witness
to the events that transpired, the following is true regarding the MUNICIPALITY’S alleged
claim (Complaint # AB123456) of a dispute between FRISTNAME LASTNAME and SOME
MUNICIPAL COURT, SOME COUNTY and its Authorized Representative, POLICY OFFICER
(henceforth Claimant).
The person referred to as FRISTNAME LASTNAME resides at 123 W. FAKE DR.,
STRAWBERRY, AZ 12345. On March 31st, 2011, while traveling northbound on Some Road
near Some Street in Strawberry, AZ, a photo enforcement camera allegedly recorded
Affiant’s conveyance’s speed as 47mph in a 35mph zone at. The person known as
FRISTNAME LASTNAME is aware of the speed limit and the photo enforcement cameras on
Some Road near Some Street in Strawberry, AZ. Affiant attests truthfully and with full
knowledge that FRISTNAME LASTNAME has traveled this same highway 3-4 days/week since
05/2008 and has always obeyed the posted speed limit. Affiant insists the living, injured
party present at the time and place of the alleged incident be produced to justify Claimant’s
alleged claim. Further, please produce a copy of the Designation of Authority Order (DOAO)
granting Claimant authority to pursue the matter at hand. Affiant demands the requested
evidence be produced and sent to Affiant within 10 (ten) days of receipt/ date of this
affidavit or Claimant will be considered held in default and the matter dismissed and
settled. This Affidavit is being sent to the address provided by the Claimant (140 E. Poser
St., #150, Strawberry, AZ 66600) Certified Mail, tracking # 1234 1234 0001 1234 1234.
