During the summer months, your check will not be mailed, but will be deposited directly
with the stub available online at www.minidokaschools.org under iVisions employee portal.
If you “opted out” of direct deposit, you can pick up your check on the official pay date at the
District Service Center (District Policy # 810.96).

1. Grade and attendance records completed on both


the report card and cumulative folder. ______

2. Grade books containing attendance records turned into the


Principal/Director. _______

3. Books, equipment, and supplies inventoried and properly


stored for summer. Lost student textbooks paid for. _______

4. Keys checked into principal. Room left neat and orderly. _______

5. ID Badges submitted to principal, director or supervisor _______

6. Books or equipment needing repair boxed and turned


in to the principal. _______

7. List of student retention recommendations turned in to the


principal or counselor. _______

8. Cumulative folders bundled and turned into the principal for


storage or transfer. _______

Note: The principal of the building shall check off the above items and keep on
file at each school/department.

Reminder: All credits earned for educational experience must be turned into the District
Service Center on or before September 15th of each year.
