List of Supporting Documents

(A) All Financial Aid applicants are to submit a photocopied version of the following documents, where
applicable. Only original copies of Form A1, A2 and B are acceptable.
Document: Compulsory in respect of :
1. Identity card or birth certificate or document certified by All applicants and household members staying with applicants

2. Latest salary slip or latest IR8A Form / income tax All employed (including self-employed) family member(s)

3. Declaration Form A1 Self-employed family member(s)

4. Letter from employer stating gross monthly salary Employed (excluding self-employed) family member(s) who are
not able to produce documents in (2).
5. Retrenchment letter stating retrenchment benefits Retrenched family member(s)

6. CPF/EPF withdrawal statement and/or pension Retired family member(s)

7. Last drawn salary slip or letter from former employer Unemployed family member(s) (excluding housewife and
and Declaration Form A2 student)

8. Insurance claims Family member(s) who have received insurance payment

9. Medical report from a certified general practitioner / Family member(s) suffering from any of the following:
specialist doctor and latest month medical bill(s) - Critical or terminal illness
- Physical or mental disability
10. Death certificate Deceased immediate family member(s)

11. Notice of bankruptcy Family member(s) declaring bankruptcy

12. Divorce or separation deed and alimony documents Applicant’s parents who are divorced or separated

(B) In addition to the above, applicants who reported no (zero) family income will need to submit
Declaration Form B (Expense Worksheet) together with a photocopied version of the following
documents, where applicable:

1. Latest utilities and phone bills
2. Vehicle loan agreement or payment advice (if applicable)
3. Latest insurance premium payment advice (if applicable)
4. Latest credit card bill (if applicable)
