the scholarship on the basis of inaccurate or false information may be expelled.

4. If you click the [Logout] button, the information provided below will not be saved.

submit this application, please check the statement of declaration and click the [Confirm and Submit]

Upon submission, you will receive an acknowledgement email with your online application number.
Please send certified copies of the following items directly to the Singapore Ministry of Education before
5 April 2011:
- Birth Certificate;
- Identity Card / Passport (if any); and
- Final Year Examination Result Slips for the last 2 years.
