ACKNOWLEDGEMENT

It is indeed exemplary to finish a task with effectively and efficiency. The tasks that were achieved by the presenters would not have been possible if it were not because of hard work and priceless efforts. In gratification, the presenters would like to make known the following entities: First and foremost, the Lord God Almighty. For never ceasing to shower the presenters with unending benison that they deserve and for granting them a case which they could present with confidence and dignity, Second, Sr. Mila Grace A. Silab, SPC - Dean of the College of Nursing, St. Paul University Dumaguete Rehabilitation Center, Third, the Talay Rehabilitation Center staff and maintenance for their enthusiasm in imparting their skills, knowledge and attitude upon working in the area, Fourth, the St. Paul University Dumaguete College of Nursing faculty and staff for their countless efforts of teaching the presenters during their long hours of lectures and for enhancing their skills, knowledge and attitude during hands-on clinical exposure, Fifth, to their Clinical Instructor Ms. Michelle Marie Manginsay, R.N. for her remarkable guidance upon the making of this clinical paper and for her efficient supervision to help enhance the completeness and comprehensiveness of the said case, Sixth, to Mr. J.C. and his significant others for their unwavering acceptance to the presenters request of making her their subject for the clinical paper.
