Severity/Consequences: 1. Catastrophic (incident causing death, widespread occupational illness, loss of facilities, reportable environmental impact) 2. Serious (severe long term injury or illness, serious property, equipment or environmental damage) 3. Moderate (injury requiring modified work duties or medical treatment, moderate property, equipment or environmental damage) 4. Minor (minor injury, requiring first aid or less, minor property or equipment damage and no environmental impact) Likelihood of Harm/Probability: A. Frequent likely to occur repeatedly and soon B. Probable likely to occur C. Remote could occur at some point but not likely D. Improbable unlikely to occur
LEVEL 1
Acceptable Risk A hazard that has been identified and can be rectified without work stoppage, has not caused injury and does not change procedure. Examples: wood with nail sticking out on walkway, poor housekeeping. A hazard that has been identified that may require a short work stoppage, may have the potential to cause incident and will require minor controls or procedural changes to continue. Examples: not following policy or procedures, not wearing appropriate PPE. A hazard that will result in work stoppage until controls are in place to reduce or eliminate risk. Examples: thin ice, faulty pressure relief valve

Caution: Risks can be reduced with further controls

LEVEL 3
Unacceptable Risk Management notified
