Sub.: Claim for final settlement of EPF, case of Shri RITESH KUMAR, EPF A/c No. DL/20659/856 Sir, I had worked at DELHI METRO RAIL CORPORATION LTD., for the period from 23.12.2003 to 17.11.2005 and I resigned from service on 17.11.2005. I tried to contact DELHI METRO RAIL CORPORATION LTD. for submission of application for the final settlement of my EPF account but could not get responded and efforts gone in vain. Thereafter as per guidelines provided on your website www.epfindia.com, I am hereby submitting my final withdrawal forms form 19 (EPF Scheme 1952) and form 10-C (EPS 1995) for claim of final settlement towards my EPF Account No. DL/20659/856 and the same is attested by a Gazetted Officer, who is authorized official for attestation as per instruction serial no. 12 for filling up of the forms. In view of the above it is requested that my application may be processed for final settlement and oblige. Thanking you Yours faithfully
