    Baseline Medications ( including duration of interruption of antiplatelets/anticoagulants )

    ALLERGIES

    Peroperative Course:

    General Anaesthesia/ Spinal Anaesthesia/ Regional Anaesthesia Induction: Cormack and Lehanes Grade Any other difficulty in intubation or ventilation including bag and mask ventilation?
