Assessment Terminology Eye Inspection I.

Visual Acuity Near Vision

Normal Findings Appropriate response to environment; Able to read 14/14 on Rosenbaum card in R eye only, L eye only and both eyes together with eyes at a 12-14 inch distance from card While client is standing 20 ft. from Snellen Chart client can read the line numbered 20 with R, L and both eyes. Recorded as 20/20

Abnormal Findings Unable to read Rosenbaum card accurately (at less than 14/14 for R eye , L eye, or both eyes. In general deficits can be called hyperopia (farsightedness) While client is standing 20 ft. from Snellen Chart client can only read the line numbered 25, 30, 40, etc. with R, L and both eyes Use line number that client cannot read more than 50%. Recorded as 20/40. In general deficits can be called myopia (nearsightedness) edema, puffiness or sagging; yellow tinted lesions (xanthelasma); Lacrimal ducts palpable and tender thin and do not extend beyond temporal canthus yellow tinted lesions (xanthelasma); discharge; Eyelid covers more than top edge of iris on one or both eyes; or palpebral fissure is asymmetric bilaterally; Inverted or everted eyelids Erythema (red) or exudate; swelling or edema; localized bright red surrounded by normal white yellow or green; rust color near insertion of medial rectus muscle; small red blood vessels; medium blood vessels;

Far Vision

II. External eye inspection Orbital

Lacrimal ducts non palpable, nontender;

Eyebrows Eyelids

Moderate hairiness; extend beyond temporal canthus Eyelids are equal in movement; palpebral fissure is symmetric bilaterally; eyelids close entirely; eyelids lie smoothly on orbit with eyelashes curled upward; No discharge; Pink, clear, non-tender; no swelling or edema White in color; smooth surface; moist; Sclera is not seen above iris in relaxed position;

Conjunctiva

Sclera

Cornea

Clear; avascular; blink reflex to corneal reflex; light shine tangentially shows no redness Pattern visible; symmetric color and size; round shape;

Iris

Presence of blood vessels; white arc or circle present around periphery; no blink in response to corneal stimulation Iris color differs; irregular shape
