    What Happens to patients after discharge? How do they maintain their level of wellness? Are they able to do the discharge instructions given to them? Did their condition get better or worse?

    Introduction
    Our study is about patients who have been discharged from the hospital, and how they maintain the level of wellness they have attained under the care of our selected institution. It is also about the discharge plan for clients and its effectiveness once the client is out of the institutions care.
