CIGNALinks Middle East builds on the strong existing relationship between CIGNA International and the Saudi Arabian Co-operative Insurance Company (SAICO), a prominent local health care administrator. On the next page there is a brief description of how CIGNALinks Middle East works. In addition, the Question & Answer section in this document will answer many of your questions. Because of local regulations and other local considerations, your benefit plan in the Middle East will differ somewhat from your standard CIGNA International coverage when accessing in-network care. In-network benefits are covered at 100 percent, according to the terms and conditions of the benefit plan. HOW THE PROGRAM WORKS


CIGNALinks Middle East is an exciting, innovative approach to health care coverage. By working with SAICO, a prominent local health care administrator, CIGNA International can provide both global reach and unparalleled local strength. Few other global insurers have such a relationship with a local health care administrator in the Middle East. As a CIGNALinks Middle East member, you will have two ID cards one from SAICO for use in Saudi Arabia, the United Arab Emirates, Bahrain, Qatar, Kuwait and Oman, and one from CIGNA International, for use when you or your covered family members access care anywhere in the world outside these countries. In the Middle East, providers normally require payment of 100 percent of charges at the time services are rendered. CIGNALinks Middle East eliminates this potentially burdensome requirement. As a CIGNALinks Middle East member, you simply go to a network provider and present the SAICO ID card. This will allow all covered medical services to be paid at 100 percent, according to the terms and conditions of the benefit plan. There are no claim forms to file. In-network pharmaceutical coverage works the same way. Out-of-network, the benefits will be those of your standard CIGNA International plan. You will need to pay the provider and file a claim with SAICO or directly with CIGNA. Dental and vision services are all out-ofnetwork in the six countries in CIGNALinks Middle East, so you should pay your provider and file a claim with SAICO or directly with CIGNA. When you are outside the Middle East, you will receive benefits directly through your global plan with CIGNA International by using your CIGNA International ID card. You can contact CIGNA International to arrange for us to pay an out-of-network provider directly, or you can pay for services in full and file a claim for reimbursement. Why did CIGNA International ally with SAICO to develop CIGNALinks Middle East?
The key to the CIGNALinks concept is an alliance with a local company. In 2002, CIGNA International entered into an arrangement with CareCard, a local administrator in Saudi Arabia, under which CareCard managed claims incurred in Saudi Arabia by members of select North American and European companies. In 2005, the Saudi Arabian Insurance Company (now the Saudi Arabian Co-operative Insurance Company) purchased CareCards Saudi operations, and the name was changed to SAICO. CIGNALinks Middle East builds on the prior program to offer medical coverage in six countries. Companies can cover employees inbound to the Middle East and outbound from the Middle East, key local nationals located in the six countries (Saudi Arabia, the United Arab Emirates, Bahrain, Qatar, Kuwait and Oman), and their families. The use of SAICOs established local networks helps to ensure quality, cost-effective care. In addition, CIGNA Internationals Health Services and Wellness team works with SAICOs medical staff to make sure that the member receives the appropriate level of care in an appropriate setting. This can involve such measures as recommending that a patient move from one hospital to another when the second has greater expertise in treating a particular kind of illness or injury. Few other global insurers have such a relationship with a local health care administrator in Middle East.

What are the advantages of CIGNALinks Middle East over my companys standard coverage from CIGNA International?

When you are outside the Middle East, you will receive benefits directly through your global plan with CIGNA International. You use your CIGNA International ID card; and you can contact CIGNA International to arrange for us to pay the provider directly, or you can pay for services and file a claim for reimbursement. Are there any differences in the coverage provided by CIGNALinks Middle East and by my companys global plan with CIGNA International?
CIGNALinks is a service enhancement to your standard CIGNA International plan, giving you access to SAICOs administrative services and its provider network. Because of local regulations and other local considerations, your scheduled in-network benefit plan in the Middle East will differ somewhat from your standard CIGNA International coverage. In-network benefits are covered at 100 percent, according to the terms and conditions of the benefit plan. Out-of-network, the benefits will be those of your standard CIGNA International plan. Persons covered by CIGNALinks in the Middle East are covered by the standard CIGNA International plan when they access care out-of-network or outside the area (for instance, if they travel to another country to receive care).

What if my local provider in the Middle East is not part of the SAICO network?
Chances are your provider is in the SAICO network. You can check with SAICO Customer Service use the number on your ID card. If the provider is not in the network, you will need to pay the provider and then submit your bill to SAICO or directly to CIGNA. Out-of-network, the benefits will be those of your standard CIGNA International plan.

Im in Saudi Arabia, but my family has stayed in our home country. What coverage do they have?
Your covered dependents will be in the same plan that you are in CIGNALinks Middle East. However, as noted above, there are two parts to the plan the part in the Middle East, and the part that covers the rest of the world. Your dependents who are in your home country, or another country outside the Middle East, will be covered by the second part of the plan. They will access medical care using their CIGNA International ID card; they can contact CIGNA International to arrange for us to pay the provider directly; or they can pay for services and file a claim for reimbursement.

Savings
If your standard CIGNA International plan requires copays or coinsurance, in-network services in the Middle East will save you money because covered medical expenses are paid at 100 percent in network, according to the terms and conditions of the benefit plan. You will not owe the provider any money. Nor will you see a bill or have to file a claim form.
