STATEMENT OF THE PROBLEM

1. Statement of the Health Problem/ Community Diagnosis: *Unhealthy dietary habits related to consumption of a more energy-dense diet, nutrient poor diet high in saturated fats, salt and refined carbohydrates as a high risk factor for hypertension leading to cardiovascular diseases 2. Statement of the Health Problem/ Community Diagnosis: * The certain group of people of those who practice sedentary lifestyle or physical inactivity are at high risk of hypertension leading to cardiovascular diseases 3. Statement of the Health Problem/ Community Diagnosis: * Failure to utilize community resources for health care, as a health deficit.

Statement of the Health Problem/ Community Diagnosis: *Unhealthy dietary habits related to consumption of a more energy-dense diet, nutrient poor diet high in saturated fats, salt and refined carbohydrates as a high risk factor for hypertension leading to cardiovascular diseases Goal: * To reduce and control the risk factors associated with unhealthy dietary habit consumption of the hypertensive group of people in Purok 5 Objectives Within 3 months of community nursing health intervention the hypertensive group of people in Purok 5 which practice unhealthy dietary habits shall actively participate & demonstrate the following as evidenced by: 1. Participate & interact with the student nurses & a dietary nutritionist (if available) in planning a healthy dietary regimen Intervention Plan Strategies/Activities 1. Shares his/her ideas & own opinion to the student nurse regarding the planned dietary regimen 3. Agrees with the student nurses proposed dietary regimen appropriate to their budget: root crops, fruits, vegetables found in their place 4. Improved BP during the follow-up home visits of the student nurses

3. Statement of the Health Problem/ Community Diagnosis: * Failure to utilize community resources for health care, as a health deficit. Goal: * At the end of the COPAR rotation, the group of Hypertensive clients in Purok 5 will be able to acquire skills, gain knowledge, manifest desirable attitudes and values in utilizing resources in the community, to prevent and detect disease, and to improve and enhance health status. Objectives Within our 3 months of nursing intervention, the group of Hypertensive clients in Purok 5 will be able to utilize community resources as evidenced by: 1. Participate actively in making a plan of action about utilizing community resources: Approaching Brgy. Health workers if the need arises, such as the onset of B.P. Visit to Brgy. Health Clinic at Palinpinon for health advises and to allow people in Purok5 to utilize or Intervention plan Strategies/Activity 1.Community Assessment -Roam around Purok 5 and look for health problems.
