IV. FUNCTIONAL HEALTH PATTERNS Before hospitalization The patient had history of hospitalization due to car accident, 4 yrs.

ago. Patient had cough 3wks ago. The patient maintains his personal hygiene by taking a bath every day, brushing his teeth twice a day. The patient was taking some food supplements like vitamins (centrum) and energy drink. Patient has dentures. His wound heals for a week. The patient eats all kinds of food. He eats 3x a day, 3-4 cups of rice per meal. He has a good appetite and likes to eat rice, vegetables, fish, fruits and sometimes fatty, sweet and salty food. The typical daily fluid intakes are 1 cup of coffee per day, 1 bottle of cola drinks 2x/week and water. He drinks 810 glasses of water a day. The Pt. defecates at least once a day or sometimes once every 2 days. His stool is soft in consistency and light-brown in color and sometimes with blood due to his diagnosed hemorrhoids stage I. The patient urinates frequently (6-7 times a day), amber in color without any odor. The patients forms of exercises are jogging, push-up and sit-up. He also considers walking as his form of exercise. He is the one who cook food. His activities are watching TV, listening to radio and some texting. During hospitalization This is not the first time that the patient was hospitalized. He maintains his personal hygiene care but still he is in complete bed rest. Patient is taking vit. B complex which is recommended by his doctor. He also hopes that he will get fast recovery. The patient eats 3 times a day and still in a good appetite. He eats fruits and vegetables. He drinks 9-12 glasses of water a day. He avoided coffee and cola drinks.

He has difficulty in defecating due to his hemorrhoids but he defecate 1-2x/ day. He frequently voids of about 7 to 8x a day, amber in color without any odor.

D. Activity Exercise pattern

He just stays at his bed. He is in CBR and scheduled for PT.

E. Sleep Rest pattern

He has difficulty in sleeping. He sleeps 4-5 hours a day, irregular sleep. He usually sleeps at 2 am and wakes up at 6 am. He also takes his nap every afternoon. He watches television while he is resting.

His sleep hour has increased to 5-6 hours a day. He sleeps every time he wants. He comforts his self by listening to soft music, to relax him and for easy sleeping.

F. Cognitive Perceptual pattern He can do whatever is instructed to him. But he has problem to his right ear, he heard a drum sound-like without pain, he diagnosed of otitis media. He doesnt use eye glasses but according to him, his vision acuity is 20/10, consulted to his ophthalmologist. G. Self-perception, Self-concept pattern He views himself as a kind, industrious, patient and peace loving person. He expresses worry about his condition and concern to his family but states no difficulty in dealing with his condition. H. Role-Relationship Pattern His family is intact and close with each other. Conflicts arise between them but they manage to resolve them easily. He has a good relationship to his family, friends, neighbours and relatives. I. Sexual-Reproductive Pattern He is still single. No problem to his reproductive system. J. Coping Stress/ Tolerance Pattern According to him, he seeks his mothers advice whenever he has problems. He is very anxious of his condition. He copes with his condition by taking enough rest and sleep and by following his doctors orders. K. Value-Belief Pattern He is a Roman Catholic. He sometimes attends mass religiously and he use to pray before going to sleep. He doesnt believe in any superstitions but he respects these beliefs. He perceives God as the center of their lives and the source of strength whenever he is not in the state of good health.
