This Supplemental Application Form is to be used by persons or entities that are already registered as Direct WESM Members (Generator or Customer), and would like to register also as Ancillary Services Provider. Those which are not yet registered as Direct WESM Members shall use the Application Form for Direct Members. Please fill in this form as completely as possible. If information required is not applicable to your organization, please indicate n/a or not applicable. Submit this Application in two (2) printed and (1) electronic copies. Applications may be submitted via e-mail but the original printed copies shall likewise be submitted. Submit all supporting documents with your accomplished Application Form.
