North Texas Forty Application for Consideration of Membership

******APPLICATION IS DUE FRIDAY, SEPTEMBER 21, 2012 AT 4PM: Please return application to the NT40 box in the Student Activities Center (Union room 320) or e-mail completed application to fortyrecruitment@gmail.com Requirements for Membership:

-Must be enrolled at UNT for at least one semester prior to selection. -Have a minimum of 60 undergraduate hours at the beginning of the fall semester in which membership begins. -A minimum cumulative GPA of 2.8 from UNT. -Be or have been a member of at least two other registered student organizations and must currently be in at least one other registered student organization. -Have been involved in one registered organization holding a leadership position. -If meets neither of these qualifications, write an essay describing why you believe you are deserving of membership. -If the student is a graduate student, he/she must have attended UNT as an undergraduate, or have enrolled at the University one semester prior with a cumulative UNT GPA of 3.0 prior to the selection.

Upon completion of this application for the North Texas Forty Student Foundation (NT40), please be aware of the following commitments:

Membership in NT40 requires a minimum of two semesters. Three semesters is required in order to receive a graduation stole. General body meetings will be held at 8pm on every other Sunday. All meetings are mandatory. Members are required to sit with NT40 during designated mandatory football games (Fall semester) Members are required to attend designated mandatory Alumni Association events. The primary focus of NT40 is to promote the student-to-alumni life cycle while exemplifying campus and community leadership. All members must work two 1 hour shifts at the Alumni Pavilion tailgate throughout the fall semester. These shifts occur before the 5 UNT home football games. Only 6-8 members may sign up for each shift. Shifts are designated on a first come, first serve basis. Members will be required to serve on at least ONE internal committee in NT40. Each member shall exhibit leadership, character, and qualities appropriate for representing UNT and NT40 in a consistent, willing, and positive manner.

Please indicate four times you are available for an interview between the hours of 8a.m. and 5p.m., in one-hour intervals (ex: 3pm-4pm) on these dates. NT40 will then be in contact with you in order to set up an interview time.

This should be completed in addition to the interest question above.
