METHOD STATEMENT

Construction of Haulers Lounge at Holcim Bulacan Plant, Bgy. Matictic, Norzagaray, Bulacan by Lambino Enterprises The Method Statement describes the management arrangements and systems of work to be adopted by Lambino Enterprise during the Construction of the Haluers Lounge at Holcim Bulacan Plant compound. This document, together with the Risk Assessment will suffice for the majority of works to be undertaken for the successful completion of this project. Certain procedures are outlined herein for controlling cost, quality, schedule and various functions of how work will be coordinated and managed. 1. MOBILIZATION a. Immediately upon receipt of Notice to Proceed, Lambino Enterprise will submit detailed working drawings, specification, design and analysis, all in accordance with the National Structural Code of the Philippines and the Holcims preliminary design brief for Holcims ap[proval. All materials to be used will be meeting the Philippine Standards and is subject also to the approval of the Holcim engineering department. b. Approved drawings and specification, bill of materials and cost estimate, structural design and analysis will be submitted to the Municipal Engineers office of Norzagaray, Bulacan as pre-requisite in acquiring building permit for this project. c. Temporary construction facilities, material lay-down yard, and field office complete with porta-toilet and lighting shall be erected on site designated by Holcim representative. d. Determine control points and datum lines with reference to the benchmarks established by Holcim. e. Set staking, check elevations and set stations based on established control points. 2. EXCAVATION WORKS a. All excavation work will be carried out according to the safe systems of work laid down in the Safety Policy of Holcim. b. Whenever reasonably practicable, the preferred method of ensuring safety in excavation will be by battering or benching. c. Excavation shall meet the factor of safety recommended by the Bishop, Janbu, Friction method, or method of slices in determining the soil stability around the excavation pit. d. Drag boxes or other proprietary support systems will be utilized where benching, battering, and shoring or combination thereof is deemed unsuitable or insufficient.

e. A supply of other shoring materials will be available for instances where neither of the above method for preventing collapse are suitable. f. Ladder will be secured in convenient locations within all excavations where there is no other safe means of access/egress. g. Lambino Enterprise will supply sufficient physical barriers for erection around excavations greater than 2 meters in depth. Excavation less than 2 meters in depth will be surrounded by bunting or other suitable material. Lambino Enterprise will be responsible for the erection and maintenance of these barricades. Reflectorized warning devices and signages will be installed at conspicuous location as necessary. 3. CONCRETING WORKS a. Reinforcement meeting the NSCP requirement will be purchased, deliver on site, fabricate and installed on site in accordance to alignment and levels shown on approved drawings. This includes but no limited to column footing, wall footing, columns, tie beams, roof beams and ground slab. Kitchen counter tops shall also be concrete slab to receive tileworks. b. Formworks will be fabricated and installed to produce structural elements called for by the approved drawings. c. Columns side forms will be stripped after 24 hours of pouring, suspended beams after 14 days. d. Concrete curing shall be carried out for 3-days. e. Prior to slab on grade reinforcement and concreting, necessary filling works shall be in 150mm layers compacted to 95% relative density. Gravel bedding shall be in-placed, install reinforcement, then concreting according to plans. f. Anchor bolts shall be set-out prior to roof beam concreting to receive truss base plate. g. All embedded items shall be in-place prior any concreting works. 4. MASONRY WORKS a. Footing reinforcement will be laid, then concreted, true to its required level. Then masonry blocks of required thickness follows reinforcing in accordance to standard reinforcement. b. Plastering follows. Lintels and opening reinforcement in-placed. 5. STRUCTURAL WORKS / ROOFING WORKS a. All pre-fabricated structures will be shaken up at the jobsite material yard. b. Damaged on painting of pre-fabricated steel works shall be repaired as necessary. Finished painting shall be done as per Holcim specification or standard painting specification. c. Check the steel work settings and alignment before grouting works.

d. Welding shall be in accordance to American Welding standards and NSCP. e. Approved roofing materials and insulation on approved purlins shall be used, installed according to acceptable workmanship. Roofing accessories shall be metting the approved specifications. 6. PLUMBING WORKS/DRAINAGE WORKS a. Only qualified plumber shall be utilized for this job. b. Work shall; be in accordance to National Plumbing Code of the Philippines. c. Design shall likewise follow the above mentioned code. d. Alignment, levels and workmanship shall be of highest standards meeting the Holcim requirements. 7. ELECTRICAL WORKS a. aAt the early stage of the project, electrical trenching works and other underground activities shall be closely coordinated with civil works schedule. b. Qualified electrical supervisor, engineers, foreman and craftsman will be assigned to the electrical works. 8. FINISHING WORKS/DOORS AND WINDOWS a. Painting shall be done by a qualified painter, skilled to carry out the work. b. Tile works shall be carried out by skilled tile setter experienced in doing the same type of works c. Doors and windows shall likewise be done by qualified installer.
