What is the AchieveAbilities Program?

The AchieveAbilities program targets young adults with mild to moderate intellectual / cognitive disabilities, who are seeking to explore and develop vocational and life skills in preparation for their transition to adult life. In partnership, CareChoice and Care Training Institute are thrilled to announce the launch of an innovative new program that has a practical and holistic approach to transition training designed for young adults. Through vast expertise in the Disability and Community Services Sector we have discovered significant gaps in the resources available for young adults wishing to explore and develop social, living and vocational skills prior to entering the adult world of further education, employment and AT&SS. My daughter is interested in finding a job, but has no idea how to start how do I help her? It has been suggested my child goes straight into employment, but I dont feel she is ready what else is out there? My son left school 2 years ago and is now lost in the system, who can help him? I want my daughter to have the opportunity to build a vision for her future, yet I feel she has been given no options to make an informed decision

Goal Orientated & Outcome Driven

AchieveAbilities is a goal orientated, person centered program, designed to identify the individuals personal pursuits within a supportive and structured learning environment. The goals identified are continuously reviewed throughout the year to ensure the individual stays on track, maintains focus and motivation, and ultimately, is able to achieve their own personal outcomes.

What are the common concerns for young adults and their families?
When a young adult with special needs transitions into the world of adult life, it is important to maintain or find the support services that is of most benefit and specific to the individuals needs. Common questions asked include; What are the options for my son after he graduates from high school? Are there more options for my son than just day program?

Program Features:
A unique program empowering individuals to explore and develop their own set of future goals. A self directed program with the focus being on the individual. An ongoing and continual review process with an instructor to ensure identified goals are achieved. Delivered by industry professionals who have an understanding of the barriers faced by our participants. A structured and supportive learning environment that facilitates growth and independence.

For many individuals, employment may not be an immediate priority or something that they are psychologically ready for.
