CONTRACT OF SERVICE

KNOWN ALL MEN BY THIS PRESENTS: This contract is made and executed this 5th day of October, 2012 at G.M.A., Cavite by and between.

SAN JOSE WATER SERVICE and DEVELOPMENT COOPERATIVE represented by the Manager Mr. Antonio dela Paz herein referred to as a first party;

And

Miguel Corralejo of legal age, Filipino and resident of 555 Moonlight Hill G. de Jesus, G.M.A., Cavite referred to as a second party:

WHEREAS, the second party has entered a contract with the first party for the PAINTING AND REPAIR WORKS of SAJOWASEDECO ground facility.

SCOPE OF WORKS:

1. Mounting of scaffolding using coco lumber. 2. Surface preparation includes surface cleaning, paint scrapping, putty application, etc. for the following area. 2.1 Outside and Inside walls fronting Carolinos residence (Ground Floor and Second Floor). 2.2 Outside wall only fronting market site (Ground Floor and Second Floor). 3. Application of AQUA GUARD PAINT WATER Proofing on the above mentioned walls. 4. Dismantling of Scaffolding and cleaning of the affected area.

GENERAL NOTES AND CONDITIONS:

1. All works shall be done in neat and workmanlike manner. 2. All materials, consumable items and tools needed on the said painting works shall be supplied by the contractor. 3. Caution signage shall be provided by the contractor to be placed on the area as needed. 4. The CONTRACTOR shall provide and used safety gear specially when working on higher or elevated area. 5. The CONTRACTOR shall ask permission first from the occupants or manager before proceeding of the painting works specially on the inside wall. 6. The CONTRATOR shall be responsible for the workers salaries and wages incurred on the said painting works.

7. The CONTRACTOR shall be liable for any damages on site and/or on the property of the owner as the result of misconduct, negligence and other failures upon execution of their works. 8. The CONTRACTOR shall call the attention of the OWNER before proceeding additional works or works not included in the contract. WHEREAS, for the consideration of the contract price for the cost of LABOR and MATERIALS is TEN THOUSAND (Php 10,000.00) PESOS ONLY. WHEREAS, down payment of fifty percent (50%) will be given before the work scheduled and fifty percent (50%) will be given upon the completion of work.
