1.0 Project Background

Andhra Pradesh (AP) state has one of the longest coastal belts in India. Out of 67,362 Km of road network in the state, the nine coastal districts cover 26,000 km of road length. Every year during the period of April-May and September-December, cyclonic situations set in to the Bay of Bengal. With the recent changes in climatic conditions, the catastrophic events are increasing resulting in very devastating cyclones like Laila, Jal etc where the loss is enormous. In the 97 years ranging from 1892 to 1989, there were as many as 67 cyclones within the State. It is essential to provide safe residential accommodation to the vulnerable population during any eventuality or disaster due to natural calamities such as cyclones and associated tidal waves and storm surges. The project is formulated based on experiences of Government of Andhra Pradesh in mitigating such disasters during the past and also keeping in view the existing facilities available in the project area in Andhra Pradesh. The Government of India has applied for a credit of US$ 255 Million equivalent from the International Development Association (IDA) toward the cost of National Cyclone Risk Mitigation Project (NCRMP), and it intends to apply the proceeds to payments for goods, works, related services and consulting services under this Project. National Cyclone Risk Mitigation Project (NCRMP) Project Implementation Unit (PIU), Andhra Pradesh, headquartered at Hyderabad has invited the consultants to submit proposals for providing the consultancy services for Third Party Quality Audit for 9 coastal districts namely Srikakulam, Vijayanagaram, Visakhapatnam, East Godavari, West Godavari, Krishna, Guntur, Prakasam and Nellore. The project Construction of Multipurpose Cyclone Shelters, Roads to Cyclone Shelters, and Roads to habitations and Roads & Bridges under NCRMP is funded by International Development Association and received through Government of India. This consultancy is envisaged to audit the quality of works being implemented by various line departments working under PIU. The number of works under progress / for which agreements are concluded in each district is shown in the table below.

Progress of NCRMP works:

The district wise progress of works is shown below. The works in the West Godavari are not started and the works in Krishna District has just started. At many places the first milestone is completed but works are not started. The line departments are to analyze the reasons for the slow progress and if necessary penalty for the slow progress as per the agreement conditions should be imposed.

Works to be done as per Common Milestone for Two works in this Package: Mile stone 1 to the end of Dt: 26.07.11- Earthwork, CD Works and GSB. Mile stone 2 to the end of Dt: 26.10.11- WBM Gr-II, Gr-III and Shoulders. Mile stone 3 to the end of Dt: 26.01.12- BT and Road furniture. The formation widths and Earthen shoulder widths are verified from Km 0/0 to Km 0/775 and at some locations shoulders have slipped and the same shall be rectified. It is observed that the shoulder material of sand and gravel is not properly mixed. It shall be scarified, suitably remixed, watered to OMC and compacted to achieve the required relative compaction of min.98% of lab MDD as per the agreement clause. At some locations the formation width is observed to be less than the estimated width. The field officers are requested to furnish the reasons for the same. The measurements like width and thickness of the WBM Gr.-3 Layer from Km 0/0 to Km 0/775 is verified and found tallying with the required measurements. The details are given in Annexure - 2. Volumetric analysis tests were conducted for WBM layer from Km 0/0 to Km 0/775. It is fairly tallying To specfication of clause 405.13 of MORD. Gradation is conducted for OGPC and Seal coat CA material and results are found to be within the specified limits as per agreement specification. During execution, The rate of spraying of prime coat is verified and found to be within the specified limits as per Table 502.1 of MORD. The spraying temperatures are also observed during execution and found to be varying from 320C to 350 C, which is within the specified limits as per MORD. During execution, the rate of spreading of tack coat is verified and found to be within the specified limits as per Clause 503 and Table 503.1 of MORD. The spraying temperatures are also observed during execution and found to be varying from 38C to 42C, which are within the specified limits as per MORD specifications. During execution, Temparature at mixing, laying and at compaction are verified during execution and found to be within the specified temperature range. The obtained values of Bitumen Extraction Test Results for OGPC and Seal Coat in % as per IRC: SP: 20 and IRC: SP: 11 from Km 0/0 to Km 0/775 are verified and found tallying with the required.The details are given in Annexure - 4. White wash shall be done to parapet walls of the existing culverts for the clear visibility of vechicular traffic. Village name boards, cautionary and mandatory signboards at necessary points all along the outer edge of sharp curves and at school locations shall be erected. Some of the KM and HM Stones are missing in this working road stretch.The field officers are requested to erect them immediately.

Bitumen Extraction Test conducted for OGPC and Seal Coat as per IRC: SP: 20 and IRC: SP: 11 from Km 0/0 to Km 1/112 and the results are found tallying with the required. The details are given in Annexure - 4. The measurements like width and length of parapets and pipes of all the culverts are verified and found tallying with the estimate. Some of the KM and HM Stones are missing in this working road stretch. The field officers are requested to make arrangement to rectify them immediately. Village name boards, cautionary and mandatory signboards at necessary points all along the outer edge of sharp curves and at school locations shall be erected. Parawise compliance report shall be furnished duly attending to the above remarks.

The formation widths and shoulder widths are verified from Km 0/0 to Km 0/860 and Km 1/163 to Km 1/492 and at some locations shoulders have slipped and the same shall be rectified. It is observed that the shoulder material of sand and gravel is not properly mixed. It shall be scarified, suitably remixed, watered to OMC and compacted to achieve the required relative compaction of min.98% of lab MDD as per the agreement clause. It is observed that in between KM 1250m to 1260m the RHS shoulder width is less than (i.e 1.5m) the required width of 1.875m.It is requested the field officers to furnish the reasons for the same and rectify. The measurements like width and thickness of the GSB Layer from Km 0/0 to Km 0/860 and Km 1/163 to Km 1/492 are verified and found to be tallying with the required measurements. It is to be specified in the agrrement specification. As it was not specified in the aggrement of this work, permission from the competent authority is necessary. The measurements like width and thickness of the WBM Grade-II and WBM Grade-III layers from Km 0/0 to Km 0/860 and Km 1/163 to Km 1/492 are verified and found to be tallying with the required measurements. The compressive strength of CC Road is tested for approximate values with Rebound Hammer on dtd. 14.11.2011 and the results are varying from 29.33 to 36.33 N/mm2. Two sets of cubes tested for compressive strength of 7 days and 28 days seperately at laboratary. The average value of compressive strength is observed for 7 days as 22.736 N/mm2 and for 28 days as 30.66 & 31.99 N/mm2. The thickness of the CC Road is observed as 202 to 213mm. The Quality Control Lab is not provided at site for testing of material. The line department officers are requested to take necessary action to establish Quality Lab at site at early stage. Cores will be taken to verify depth and compressive strength after 28 days of lying. MORD Specifications and IRC: SP-62-2004, Guidelines for the construction and design of Cement Concrete roads for low volume roads, need to be followed in cutting and spacing of joints and curing the CC road as mentioned below. 1. Construction of joints shall be mechanically sawn joints spaced 2.5 m to 3.75 m 2. Length of the panel in the direction of traffic shall not be less than the width of the pavement. 3. Joints shall be cut when the pavement is neither too soft nor too hard and able to bear the weight of the machine and crew. Normly it may vary from 8 to 12 hrs depending on weather condtions Construction joints shall be butt joints and placed after a days work is over or when work is suspended for more then 30 minutes Expansion joints shall be provided near bridges and slab culverts and consists of a bitumen impregranted joints/ premoulded synthetic joints filler board about 20 mm thick. 1. After completion of the finishing operations, the surface of the pavement shall be coverd with wet Hessain cloth, burlap or jute mats. They shall be maintained fully wetted and in position for 24 hrs 2. Upon the removal of the wet covering out the end of 24 hrs, the slab shall be thoroughly wetted and cured by ponding or sprinklers. After minimum 28 dys after casting the concrete pavement ,joints groove at contraction joint shall be widened to a 10 mm width and to a depth 18 to 20mm and shall be sealed

The measurements like width and thickness of the WBM Gr.-III Layer from Km.1.100 to Km 1.825 is verified and found to be fairly tallying with the required measurements. The details are given in Annexure - 2. Volumetric analysis tests were conducted for WBM grade III layer from Km 1.100 to Km 1.825. It is fairly tallying with the specfication of clause 405.13 of MORD. QC tests conducted for WBM GRADE II (field and lab) ie Gradation, AIV, FIV and water obsorption.The details are given in QC Test summary. The results are fairly tallying with the specfications of clause 405 of MORD WBM Grade III layar was distrubed due to vehicle movement. Regular wate sprinkling shall be done before taking up BT work. Grade -II WBM layer completed and Grade-III WBM lying is in progress from Km 0/0 to Km 0/370. During inspection on 2-11-2011, oversized metal was observed and the metal was asked to be either removed or rectified by rebreaking to correct size. During inspection on 12-11-2011, it was observed that the earier insructions were complied. Gradation tests were done on Grade III metal were also done and found to be satisfactory. The thickness of WBM Grade-II verified at random locations. From the results the thickness are tallying with the required thickness The volumetric test was conducted for Grade-II metal. It is as per requirements Shoulder formation was not done simultaneously with the WBM GrII and Gr III layers. Hence it is instructed to construct the shoulders simultaneously along with WBM Grade-II &III layers. The Shoulder width's are found to be less than required as per drawing at some locations in village portion. Camber is not properly provided at some location in between stretches Km 0/0 to Km 0/370. It is instructed to maintain proper camber as per approved drawing duly using camber board and templates... Km and HM stones are not available on the the road stretch.The field officers are requested to provide the same. Cautionary sign boards at necessary points and village name boards are not provided. The project informatory boards with all details were not provided as per aggregement.The same shall be provided as per the guidelines given by PIU, NCRMP. The quality control tests shall be conducted as per MORD section 1800 and QC records shall be produced for verification by TPQA staff. Construction of Cyclone Shelter at Pedaraghavulapeta of Uppalaguptam Mandal, E.G.Dt.

As per the Agreement the Milestones are fixed as follows. Milestone 1 Ground Floor Plinth Level : 3 Months (16.01.2011) Milestone 2 First Floor Roof Slab Cast : 3 Months (16.04.2011) Milestone 3 Compleion of Works : 3 Months (16.07.2012) There is a narrow slab culvert at the approach to the cyclone shelter road. At present this is the only approach to the site. For movement of bigger vehicles, the approach needs to be widened duly constructing wing walls on four sides. Another approach can be provided by constructing a pipe culvert on the southeren side of the shelter site. Proper approaches are necessary during cyclone or other natural calamities. It is noticed that ground water table is high and required to be dewatered by suaitable beams for excavation and construction of foundation.

Mile stone 2 to the end of Dt: 29.05.12- WBM Gr-II, Gr-III and Shoulders. Mile stone 3 to the end of Dt: 29.09.12- BT, CC pavements and Road funiture. Grade -III WBM completed from Km 0/0 to Km 0/475. During inspection by Site engineers, oversized metal was observed in grade III metal collected and it was instructed to remove or rebreak the oversizes metal. During inspection on 12-11-2011, it was noticed tha the oversized material was removed and correct sized metal was collected. The thickness of WBM Grade-III layer was verified at random location thicknesses are as per requirement. Camber and longitudinal undulations were also measured and tabulated in annexure-I. The volumetric test conducted for Grade-III metal layer and it satisfied the requirements. Shoulders foramtion is in progress. Shoulder formation has to be done simultaneously with the WBM Gr III layer. Hence it is instructed to construct the shoulders The Shoulder width's are found to be less than the width specified as per drawing at some locations in village portion. Camber is not properly provided at some locations. It is instructed to maintain proper camber as per approved drawing using camber board and thickness templates. Km and HM stones are not available on the road stretch.The field officers are requested to provide the same. Cautionary sign boards at necessary points and village name boards are not provided. All the Quality Control tests as per MORD section 1800 shall be conducted by the contractor and the field officers and the test records shall be produced to the TPQA staff for verification. QC laboratory is not provided at site. The line deportment is requested to take necessary action and see that the Quality lab is established at site at an early stage. CC pavement is completed from Km 0/475 to Km 0/525. It is observed that the following MORD & IRC: SP-62-2004 guidelines were not followed regarding joints and curing in CC road construction MORD Specifications and IRC: SP-62-2004, Guidelines for the construction and design of Cement Concrete roads for low volume roads, need be followed in cutting and spacing of joints and curing the CC road as mentioned below (i) Construction of joints shall be mechanically sawn joints spaced 2.5 m to 3.75 m (ii) length of the panel in the direction of traffic shall not be less then the width of the pavement (iii) Joints shall be cut when the pavement is neither too soft nor too hard and able to bear the weight of the machine and crew. Normly it may vary from 8 to 12 hrs depending on weather condtions (iv) Construction joints shall be butt joints and placed after a days work is over or when work is suspended for more then 30 minutes (v) Expansion joints shall be provided near bridges and slab culverts and consists of a bitumen impregnated joints/ remolded synthetic joints filler board about 20 mm thick, and dowel bars (vi) Longitudinal joints shall be provided for two lane road and shall be saw cut. Curing: (I) After completion of the finishing operations, the surface of the pavement shall be covered with wet Hessian cloth, burlap or jute mats. They shall be maintained fully wetted and in position for 24 hrs

Construction of Minor Bridge at Km. 7/2 near Pulligadda of Konada Ramatheertham R&B Road to Konathalapalem via Yerukonda, Pasupam, Kollayavalasa (Z.P. Road) in Vizianagaram District
Progress: About 44.12% of work is completed as against 52.00%. The progress of work is slightly slow. Works to be done as per Milestone: Mile stone 1 to the end of Dt: 30.06.2011Earthwork for foundation,PCC for Foundation,Footings. Mile stone 2 to the end of Dt: 31.01.2012Substructure for abutments and wing walls, Supply and fixing of elastomeric neoprene bearings, Super structure and 300mm thick Grouted rough stone revetment, Earth work for approaches etc.,. Mile stone 3 to the end of Dt: 30.05.2012Super structure and construction of precast RCC railing,Streep seal expansion joints,SDBC etc. The width of filter media is verified and found between o.6m to 0.65m.

Gradation,AIV and FI are conducted for Coarse aggregate.Gradation ,F.M,and Silt content tests are conducted for fine aggregate the results are found with in the specified limits as per IS: 383-1970 table- 1 & 2. During execution, tests are conducted for Water Ph value, temparature and slump of concrete are tested and found within the specified limits in MORT8H standard specification for Roads and Bridge works. The reinforcement mat for slab and beams are verified and found to be fairly tallying with drawing details. The physical measurements for elastomeric neoprene bearings are verified and found OK. The field officers have furnished the test results for Water, steel and cement obtained from Department of Civil Engineering, Andhra University, and Visakhapatnam. The field officers are also requested to apply cement slurry to the steel bars so as to protect them from rusting. The field officers are also requested to test the samples of cement from every consignment, conduct all the required tests on cement such as cube strength, initial setting time, Final setting time, consistency, soundness etc. The field officers are requested to provide the invoices bills of the materials for verification. The reinforcement steel shall be neatly stored on wooden blocks, racks or platforms above the ground in clean and suitably marked to facilitate inspection and identification as per Cl.1603 of MORTH.

Providing B.T to the Road from NH-5 to Ramarayadupalem in Yelamanchilli Mandal

18.82 % value of the work is completed. WBM Grade- II is completed. Works to be done as per Milestone: Mile stone 1 to the end of Dt: 14.12.11- Earthwork, CD Works and GSB. Mile stone 2 to the end of Dt: 14.03.12- WBM Gr-II, Gr-III and Shoulders. Mile stone 3 to the end of Dt: 14.06.12- BT and Road funiture. The earth work done in the widening portion is not compacted properly, it is to be constructed in layers, sectioned, watered to OMC and compacted to achieve the required relative compaction of min.98% of lab MDD as per the agreement clause. The field officers were instructed to rectify the same. Site registers are not available. The field officers are requested to maintain the material registers and the same shall be furnished to the TPQA for verification. The formed width is less than the approved drawing width of 7.5m. The formed width is varying from 6.0 to 7.0m. It is observed Hand pump and electrical poles are inside the road width on both sides from CH: 0/0 to 0/390m. The field officers were asked to take necessary actions to shift them. The formation widths and earthen shoulder widths are verified from Km 0/0 to Km 0/390 and at some locations shoulders have slipped and the same shall be rectified. The measurements like width and thickness of the WBM Gr.-2 Layer from Km 0/0 to Km 0/390 is verified and found to be fairly tallying with the required measurements. The details are given in Annexure-2. Volumetric analysis tests conducted for WBM layer from Km 0/0 to Km 0/390. It is fairly tallying to specfication of clause 405.13 of MORD. Quality Control lab is not available at site. The Executive Engineer is requested to give necessary instructions to the contractor and see that the quality lab is established at site. Village name boards, cautionary and mandatory signboards at necessary points all along the outer edge of sharp curves and at school locations shall be erected. The Project informatory boards with all details were not provided as per aggrement. The same may be provided immediately at site as per the guide lines by PIU, NCRMP.
