Duty Hour Policy for Medical Students During the Second 100 Weeks The Hofstra North Shore

LIJ School of Medicine adopts the duty hour regulations followed by the Graduate Medical Education programs sponsored by the North Shore-LIJ Health System. These regulations comply with both the New York Codes, Rules and Regulations (NYCRR), Title 10, Section 405.4, and ACGME Final Requirements, Resident Duty Hours Language. Duty hours are limited to a maximum of 80 hours per week, averaged over a four-week period. Medical students must be scheduled for a minimum of one day free of duty every week (when averaged over two weeks). Duty periods of medical students must not exceed 16 hours in duration. Medical students must not be assigned additional clinical responsibilities after 24 hours of continuous in-house duty. o In unusual circumstances, medical students, on their own initiative, may remain beyond their scheduled period of duty to continue to provide care to a single patient. Justifications for such extensions of duty are limited to reasons of required continuity for a severely ill or unstable patient, academic importance of the events transpiring, or humanistic attention to the needs of a patient or family o Under those circumstances, the student must: Appropriately hand over the care of all other patients to the team responsible for their continuing care; and, Document the reasons for remaining to care for the patient in question and submit that documentation in every circumstance to the Director of ACE. Minimum Time Off between Scheduled Duty Periods o Medical students should have 10 hours, and must have 8 hours, free of duty between scheduled duty periods. Maximum Frequency of In-House Call o Students must not be scheduled for in-house call more frequently than every 4th night. Students are expected to log their hours via a web-based log. Compliance with this policy is monitored by the Director of ACE. Duty hour violations can be reported at any time to the Director of ACE. In the event of a duty hour violation, the Director of ACE, or his/her designee, investigates the circumstances of the violation to determine if the violation represents a systematic problem or a situation unique to the student. If the investigation reveals a problem in the system, the Director of ACE makes changes in the system to rectify the problem. If the investigation reveals a situation unique to the student, the Director of ACE discusses the situation with the student and his/her supervisors, and counsels them, as necessary, to prevent further violations.
