Arianna Medina RN 66 Dr. Rita Rich 7-9-12 RN 66 INTERDISCIPLINARY TEAM MEETING ASSIGNMENT 1.

Identify the staff/disciplines contributing to the team. The staff that contributes to the team is made up of psychiatrists, social workers, discharge planner, pharmacy, the lead registered nurse,and the director of the floor. 2. What are the specific roles of each team member? The lead registered nurse gives report on each patient. The psychiatrist interviews the patients daily, from this medications and treatments are adjusted. The social workers work with the patient asking living situations and working with family and other resources for when they are discharged. The discharge planner works with social workers to place them in places like sober living and outpatient programs. The director of the unit supervises the nurses on the floor. 3. What are the tasks and how are they carried out by the team? The lead registered nurse gives report on each patient. The team discusses the patients progress. The psychiatrist talks to the team about the treatments and works with the social worker and discharge planner to discharge the patient. 4. Describe three issues you observed related to the clients needs. A client issue this day was regarding medications. The team discussed changing a patients medications by mouth to injections. Another client issue is housing. Since some of the patients were previous homeless the social worker and discharge planner talked about where the patients can stay during their recovery. A third issue for the clients is their behavior during their stay and safety of the other patients and staff. This day it was brought up that there was a patient that was scaring the custodian the day before. 5. Identify barriers or challenges encountered by the staff regarding the care of the clients. A challenge regarding the care of the clients this day was ways to work with a patient that refused medication and was scaring the other patients and staff. Another challenge was speaking to one patients wife and also having that patients girlfriend wanting to be involved. 6. Describe the interactions between staff members. The registered nurse gives the most information about the patient and relays the information to the rest of the team. The psychiatrists ask a lot of questions regarding their behavior especially during the night and discuss treatment options

from there. Social workers are more involved towards the discharging of the patient. 7. Identify and discuss three positive aspects of team meeting. A positive aspect was that the team is concerned about the patients housing knowing that many of patients are homeless. Another positive aspect is that the registered nurse reports any adverse effects of the patients medications and this helps pharmacy and doctors work to prescribe new medications or different dosages. A third positive aspect was that since the meetings are daily there is an up to date progress of the patient and the doctors are there to see the patients and give orders on the spot. 8. What was your overall impression of the team process unit? My overall impression of the team was that it was very professional. The team discussed nothing but the patients and how to help them. The meeting was organized and they were able to discuss every patient in a timely manner.
