REQUISITE DOCUMENTS FOR FAMILY REUNION VISA MANDATORY DOCUMENTS FOR CHILDREN

MANDATORY DOCUMENTS (FOR ALL APPLICANTS): Please show original documents at the time of Submission of Application for Verification S.No. 1 2 3 Requirements Application form fully completed and signed by the applicant. Two recent passport-size photographs (white background). Please do not staple the photographs to the application form. Passport/Travel Document must: a) Have been issued within the previous 10 years and the validity must not exceed 10 yrs b) At least have a validity of 3 months after intended stay. c) Have 2 Blank Pages Nulla Osta (No Objection Certificate) in original, issued by competent Questura in Italy. Kindly note that the Nulla Osta is valid only for a period up to six months from date of issue. The Nulla Osta will have to be renewed on expiry. Discrepancy, if any, in the names of the applicant/s on the Nulla Osta and the passport/s, should be corrected to correspond with the name on the passport. Photocopy of the passport (all pages) of the person who has applied for the Nulla Osta in Italy. If available, also of the old passport. Photocopy of the Permesso di soggiorno (Permit of stay). Photocopy of the air ticket (only after having received the visa) Verification of original documents and of the English translation of Birth Certificate, Marriage Certificates, Ration Cards & other relevant documentation. * If any change has occurred in this regard an adequate proof of residence needs to be submitted (Bank account, telephone/electricity bill etc.) Birth Certificate of the child/children in original, issued by the Registrar of Births and Deaths, clearly mentioning the registration number, along with translation in English language, both verified and authenticated by the Under Secretary, Home Department. An Affidavit from the parent in India giving his/her consent to the child traveling to Italy for family reunion with the parent residing in Italy. The name of the person accompanying the minor should be mentioned with a copy of that persons passport and air ticket. In case the minor is traveling unaccompanied, the name of the airline should be mentioned. If both parents are resident in Italy, then a dichiarazione equivalent to the Affidavit has to be done and attested by the competent Authority in Italy. PLEASE NOTE: All documents must be submitted in original. Documents such as Birth & Marriage certificates, School Leaving Certificates must be submitted in original and photocopy. Photocopies to be done ONLY after attestation by the Under Secretary, Home Department. The above mentioned originals (Birth & Marriage Certificates and School Leaving Certificates) will be RETURNED after the visa processing. The Embassy/Consulate reserves the right to request for further documentation and to verify the documentation submitted. In this case the maximum period of 90 days fixed by the Italian law for the evaluation of this visa type will be suspended till verification is completed. COMMENTS: DATE:
