DEFINITION :
2.1 Refers to releasing from the hospital upon a clear written Doctors order. 3. POLICY : 3.1 Discharge procedures must be followed to ensure patients are discharged effectively and
efficiently .
3.2 An authorized hospital discharge shall be made by a physicians written order. However, a
patient may be discharged against medical advice. (AMA).
3.3 The Physician shall document discharge order / instructions in the patients file. 4. PROCEDURES :
4.1 Review the nursing assessment of the discharge needs. 4.2 Check the Physicians Order. 4.3 Send the medication sheet and prescription to pharmacy properly and completely accom plished by the Physician with the signature and stamped. 4.4 Explain to patient and / or relative clinic appointment, home education and health teaching. 4.5 For female and minor patients obtain signature of the husband, father, or nearest male relative of legal age to be witnessed by Social Worker on duty or Manager on duty after office hours. 4.6. Give appointment card, preliminary discharge paper and medications to the patient and / or relative with proper instructions witnessed by another staff.

4.7 The patient will be wheeled to the waiting vehicle by wheelchair or trolley if non - ambulatory. 4.8 Document fully in the Nurses Notes.
