Narrative report

February 24 2012 This day is our duty in CHN. When we arrived at the health center in Tubigon, maam Domagsang talk to one of the staff there and ask for the list of the names of the pregnant women and for the place where we assigned. So after that, maam Domagsang gave us instructions then we directly proceed to the area. It took us almost 1 hour when we reached purok 4 specifically GK area. We immediately look for maam merlinda, the kagawad to ask permission for having a home visit to pregnant women. Then I and my partner Michelle Anne Nano proceed to our assigned house. When we got there, we directly started our home visit procedure. We greeted our client and introduce our selves, explain our purpose, and start interviewing. After gathering information, we do health teachings about proper nutrition, proper waste segregation or disposal and proper breast feeding. We also do some documentation and make an appointment. After that we go ahead to find our groupmates and when we find them, we take a little rest. A minute after we start walking again and we split direction with maam Domagsang and the other group which their other area is headed near to the RR. The 4 of us were headed on our way to our assigned area. When we reached to the other house that is assigned for me and my partner, we start communicating to our 2nd client. We first greeted our client then introduce our selves, explain our purpose and do interviewing. After that we do health teachings bout proper nutrition, proper waste segregation or disposal, proper breast feeding, antepartal exercise and breathing pattern; we also do documentation and make an appointment. After we finished, the 4 of us went to the store to wait for maam Domagsang and the other group for further instructions. While were waiting, we decided to take a snack and rest for a while. We waited almost 2 hours for them to returned, and when they arrived they take a little time to take a snack and to rest. We then continue our journey and went to the next area which is in purok 5 near the edlimar. We passed through some cane field and banana field. When we arrived to the house, we greeted our client; interviewed ad assessed her and her family. We also do some documentation and make an appointment. And when we finished it, we returned to the health center to take our lunch. After we ate and rest, we record some information that we gathered from our clients and gave it to maam Domagsang. Around 3pm, maam Domagsang decided to go home.

Narrative report
March 02, 2012 Today we returned to our assigned area to do intervention. When we are going to our 1st client we met her and greeted her, as we reached their house, we rested for a while and explain our purpose for our coming back and after that we started our intervention by cleaning the house, gardening, putting a proper garbage bag and we also do some documentation. After we finished doing intervention, we say goodbyes to our client, and then we proceed to our 2nd client. When we arrived, we knocked to the door and called out if there is someone in the house, after how many attempt of calling out we decided to go to the store where the mother of our client lived and asked if someone is at home in her daughters house. When the mother of our client said that if shes just asleep in their house, we returned instantly and try calling out. Three little girls went to us and entered the house to check if someone is at home, thats the time when it confirmed that our client is not at home. So we decided to called maam Domagsang o informed her that our client is not at home. We went to the store to remained there for maam Domagsang and the kagawad to arrived, we take a snack and rest while were waiting for the arrival of the other group but 1 hour has passed maam Domagsang decided that we should proceed to edlimar where our 3rd client lived. When we arrived there, we take a little rest before we proceed for the intervention. We greeted our client and explain our purpose. Two of us do health teachings and the remaining two do cleaning around the house. After we completed the intervention, we say goodbyes to our client ad we go straight ahead to o cafeteria near the health center t take a lunch. After we ate and rest, we went to the health center for some discussions. After the discussions with the barangay captain and writing our names to their attendance book, we went home.

Narrative report
March 03, 2012 Today we returned to our area for our evaluation but instead we rescheduled it due to some problem. Yesterday, I and my partner did not make the intervention for one our client due to the reason that she is not at home so today is the time to do intervention. When we arrived to the house of our client, she is not there so we went to the store where we saw her and greeted her and also explain our purpose of coming back. When we got into their house we start the intervention by cleaning their surroundings, and placing garbage sack to proper place. We also do health teachings about proper garbage disposal, proper sanitation, and prenatal exercise and also demonstrated some prenatal exercise. After the intervention we say goodbyes to our client and we headed on our way home.
