Regular attendance is required (7:15 11:45, 1:00 5:00).


Punctuality in flag ceremony, classes and other school activities.
Present lesson plan 2 days before teaching.
Prepare presentable and functional visual aids 2 days before the teaching.
Provide your own teaching kit (chalk box, eraser, class records, school register, pentel pen,
scotch tape, and other school materials).
Use grade sheets in consolidation of records only.
Ask permission upon borrowing or using facilities.
Observe classes when you have no scheduled actual teaching.
Notify the Principals Office and Supervising Instructor when absent.
Use observation notebook.
Participate in the extension project of the SSL.
Shifting to another year level is done upon recommendation of Supervising Instructor.
Off-campus is upon the recommendation of Supervising Instructor.
Observe proper and decent dress code.
Observe decency in terms of relationship, and in dealing with teachers, students and co-student
teachers.
Respect authority at all times (teachers, parents, school officials, guests).
Language Policy should be observed at all times (in talking to students, co-student teachers,
teachers, school officials and guests).
Observe cleanliness of the assigned area and fix things ready for instruction.
Grades for on-campus teaching will be released upon clearance.

20. On-campus teaching grade is a requirement for off-campus teaching.

If incurred absences, arrange for replacement immediately.


Formative evaluation form from SI should be filed.
In actual daily teaching use Student Teaching Self-analysis form and should be filed.
Follow the stages of clinical supervision during on-campus student teaching period (preconference, observation, analyses and strategy, post-observation conference or feedback and
post-observation analysis)
25. Submit all requirements prior to off-campus teaching.
26. Submit clearance prior to off-campus teaching.

I conform to the above conditions.


I fully understand that my failure to comply with the above conditions will subject me for not
recommended for off-campus teaching.
