Midterm Mock and Final Examinations Guidelines:

Directions to the Teacher:


1. After you have created your assessment please check off each item as it applies
to your exam. For example: If your exam does not contain any fill-in-the-blank
questions, then you would not check off any items pertaining to fill-ins
2. This check list must be submitted along with your exam your department
coordinator for initial approval.
3. You may submit an electronic copy to your coordinator, but the check list must be
submitted in hard copy form.

Directions to the Department Coordinator


1. Please review the midterm, mock or final exam for compliance with the check list
items below.
2. Once the exam meets standards, check of the box below and submit the exam to
the Assistant Principal of Instruction, Ms. Spencer, for final approval.
3. The exam will be returned to you (the department coordinator), once final approval
has been given, so that you may have your department member arrange for copies
to be made for the students.

Midterm Mock and Final Examinations Checklist


1. Physical Appearance—Exams should be “copy ready”
There is a heading on the cover page (course name, teacher’s name, line for
student’s name, exam date)—see page 2 for an example
Test is typed
Test is clear and easy to read
There are no type-o’s, spelling, grammatical or punctuation errors
There is enough space between each question (if the students are to record
the answer on the exam itself.)
The pages are numbered
Each question is numbered consecutively
The visuals (drawings, maps, charts, diagrams, political cartoons, etc) are
clear
The visuals can be read by all
The same types of questions (multiple-choice, short answer, essays, etc) are
grouped together
Multiple choice questions have four reasonable possibilities that are place in
numerical or alphabetical order
The multiple choice question and answers appear on the same page

2. Directions

Clear expectations/directions are given for each part of the test for the
students
The directions mimic the state exams

3. Content
The exam material is directly from the course of study or scope and
sequence for the particular subject on a specific grade level that was taught
in class
The students’ knowledge is tested in a variety of ways including multiple
choice, fill-ins, short answer, essays, etc.
State guidelines are followed where a state exam exists

4. Final Product
The Exam meets all of the standards listed in the check list
There is an answer key, rubric or description of how the exam will be
assessed is included.
The teacher has taken the test during the allotted time frame

If you are using a scantron:

The specific scantron from needs to be requested from Ms. Spencer (via email
sspence4@schools.nyc.gov) at least 48 hours before the exam. The multiple
choice answer choices on the exam MUST be compatible with the scantron
format. For example, if the MC answer choices on the exam are 1-2-3-4, then
you will need a scantron with 1-2-3-4 choices.


