INTRODUCTION NBNC2607: Clinical Practice 14 on Renal Nursing course will focus on application of theory from the theoretical course subject NBNS3504: Renal Nursing into practicum. In order to achieve 7 credit hours, you would have to undertake BOTH the General Practicum and Specific Practicum as stipulated in the table below: CLINICAL PLACEMENT 1. General Practicum: Practicum takes place in your respective area of practice. Your official working duty or shift is considered as the practicum hour. There is no preceptor for general practicum. You are self-directed in implementing your learning activities. You are expected to apply or practice skills that you have learned from the specific practicum whenever possible. PRACTICUM DURATION 324 hours Recommended practicum hours: 54 days per semester. At least 6 hours per day and 5 days per week.

2. Specific Practicum: Placement will be in a Renal / Urology setting. You must set aside the practicum hours separately from your official working hours. A Clinical Preceptor will be assigned for you. TOTAL : 7 credits =

96 hours Recommended practicum hours: 8 hours per week in a time frame of 12 weeks.

Instruction: Please read NBNS3504: Renal Nursing for theoretical input for your practicum in addition to this module. Health assessment is an integral part of nursing. You may find it beneficial to revisit and review the module NBHS1103: Advanced Nursing Assessment to guide you in conducting assessments on clients with renal problems. Additionally, you are also encouraged to apply all the knowledge and skills gained from your previous learning, especially relating to nursing management and professionalism.

COURSE OBJECTIVES At the end of the General Practicum, you should be able to:I. Perform skills learned from the specific practicum where applicable. II. Nurse the clients using the nursing process approach. III. Communicate with clients as well as their families and relatives with caring and empathy.

At the end of the Specific Practicum, you should be able to:II. Utilise the nursing process approach in caring for clients with renal disorders and their families / caregiver. III. Acquire skills in Renal Nursing:a. Conducting initial health assessments on clients with renal disorders. b. Conducting focused assessments on clients with renal disorders: i. Assessing the renal function ii. Assessing the fluid balance iii. Assessing the electrolyte balance iv. Assessing the acid-base balance v. Assessing the psychosocial responses c. Delivering nursing care for client undergoing dialysis treatment: i. Nursing clients undergoing hemodialysis or continuous renal replacement therapy. ii. Nursing clients undergoing peritoneal dialysis. d. Delivering nursing care for clients and family/caregiver in undergoing various investigations and treatments. e. Performing various nursing procedures common to clients with renal disorders. f. Delivering health education to the client and family/caregiver with renal disorders. III. Delivering health education to the client and family / caregiver in managing the care of the client.

ASSESSMENT / EVALUATION METHODS

You will be evaluated for the course NBNC 2607: Clinical Practice 14 as follows: GRADED ASSESSMENT 1. CLINICAL PRACTICE RECORD a. Clinical practicum hours: Achieved minimum 324 hours of General Practicum with roster / record of working time attached = 2% Achieved minimum 96 hours of Specific Practicum = 2% b. Procedures log (see Section 3) Acquired at least 90% of the compulsory nursing procedures = 4% c. Clinical Conference / Bedside Teaching log Attended all Bedside Teaching sessions (4 topics) = 2% 2. CLINICAL PORTFOLIO Case Study completed and submitted by the dateline. 3. PRACTICAL EXAMINATION Final Examination on practical nursing at the end of semester TOTAL ALLOCATION OF MARKS

CLINICAL PORTFOLIO (40%) All students are required to complete a Written Assignment of a Case Study of a Client with Renal Disorder as Clinical Portfolio

Title of the Clinical Portfolio: The Nursing Care and Management of a Client with . Instructions: 1. Identify ONE Renal Client that you have nursed. Discuss your plan of care using the nursing process approach. 2. Complete the write-up of the Case Study in 3,000 words. 3. Compile your portfolio into the following arrangement:i. Cover page (soft cover). ii. Section 1: Introduction. iii. Section 2: Presentation of the case. iv. Section 3: Summary/ Conclusion. v. Section 4: References. vi. Section 5: Appendixes.

Evaluation will be based on the following criteria: a. Skilful in assessing data: Collecting comprehensive and accurate data. Performing accurate diagnostic reasoning of the data. b. Skilful in formulating nursing diagnoses and intervention: Identifying problems from data. Prioritising problems and plan of care. Setting specific, realistic and measurable objectives. Identifying nursing intervention based on relevant data. Formulating comprehensive, systematic and individualised nursing care. c. Meeting the general requirements: Logical and systematic organisation of information. Correct and appropriately referencing. Punctuality in submission of assignment. CLINICAL PRACTICE RECORD (10%) You must process the documentation of the Clinical Practice Record with your Clinical Preceptor on a regular basis. You must document the following:i. ii. Record of minimum 324 hours of General Practicum with photocopies of Roster / Record of Working Time attached. (Marks = 2%) Complete requirement of Specific Practicum: a. Achieved a minimum of 96 Practicum Hours (Marks = 2%) b. Attended ALL (4) Bedside Teaching Sessions (Marks = 2%) c. Acquired AT LEAST 90 % of the listed Nursing Procedures (marks = 4 %)

PRACTICAL EXAMINATION (50%) At the end of the semester, you will be designated to a Clinical Skill Lab in an Institution or Nursing College to sit for your Practical Examination. You will be tested on skills acquired for NBHS 1103 Advanced Nursing Assessment and a clinical specialty of NBNC 2607 Clinical Practice 14 (Renal Nursing). Tentatively 5 stations is expected (subjected to change) and it will comprised of i. Nursing Assessment ( health assessment interview / physical examination/ assessment of records or investigation results) ii. Skills specific to the clinical specialty which can be any of the combinations: a. Nursing procedures b. Clinical decision making skills c. Communication skills
