PGHA Registration for 2013/2014 Season

Registration Regardless of whether you are a returning member to the PGHA or not, you are required to complete a registration form. This form must be submitted with all required fields completed and legible in order for your child to become registered. Please ensure this form is submitted to the registrar accompanied by payment as indicated below. There are 2 payment options available - cash and cheque. Post dated cheques are accepted but please note that there will be an NSF fee charged for each cheque returned by the bank and should 2 cheques be returned, PGHA reserves the right to remove your child(ren) from further play until full payment is received. Registration/Payment Requirements Payments must be received by the registrar on or before dates indicated below in order to receive the appropriate registration fee. All registrations must be paid in full by September 15th (with the exception of Seniors). Any player that is not paid in full by this date will not be permitted on the ice for further play until all payment is received. Registration Fees:

Deposits will be made on the 15th of each month only, so please make note of that when you put dates on your cheques. A 15% discount will be given to families with 3 or more children registered in PGHA. This discount only applies to registration fees and not the fundraising fee.

Fundraising Fundraising is not optional, it is mandatory. Each family will be required to pay $100 and will receive a book of 20 raffle tickets. Each ticket is worth $5 and money collected for tickets sold is kept by the parents as these tickets are already paid for. A separate cheque is required for the fundraising and must also be paid on or before September 15 th, 2013. For anyone wishing to pay in cash installments, you will be required to complete a committed payment schedule in which you will provide dates of future payments. Should you deviate from that schedule, your child will not be permitted on the ice until payment is received. If you require financial assistance, we plan to provide appropriate links on the website to various organizations that may be able to help, but feel free to send me an email and I will try to provide you with additional information. It will however be the parents responsibility to make the arrangements with the appropriate organization because PGHA will not do that on behalf of the family. Please note that PGHA has in the past, put a cap on the number of registrations in any one division therefore it is important that everyone be made aware that registration is done on a first come first serve basis and your child may be placed on a waiting list with no guarantee of being able to play.
