This is the record of which numbers of stacked pipe pile casings (610X28mm, 9M) at the storing area was accidentally collapse near the entrance gate at WA2 of Site Office on 25 May 2012. At about 1315 pm of that day, delivering and lifting of pipe pile casings work was in progress and carrying out by a Hydraulic Truck Crane (Model no. NK-250V KATO, Reg. no. KZ4127) so as to stack the pipes at the temporary storage area at WA2 at the scene. Before the incident, the pipes were being lifted and stacked normally one by one by the crane. At the material time, some stacked pipes were struck against the lifted pipes and caused the stacked pipes for collapse and rolling down to the

Incident Report on Collapse of Stacking of Pipe Pile Casings at WA2 on 25 May 2012 hoarding. As a result, it caused damaging to the hoarding as shown in Photo 1. The pile stacking operation was immediately stopped. Subsequently, Site emergency team was informed for emergency repairing work and then arrived at the scene for investigation work. It was followed by some rectification works, which completed within two hours on the same day. 3. Cause of Incident

Afterwards, site engineers and safety officer had carried out the investigation for this incident. After having investigated, the cause of this incident could be summarised as follows: 3.1 3.2 3.3 3.4 3.5 4. The lifted pipes hit on the stacked pipes to cause collapse of pipes. Poor communication between crane operator, signalman and rigger during lifting, rigging and stacking operation. The pipe pile casings (610X28mm, 9M) were stacking highly and unsafely. The stacked pipes were inadequately fixed and secured. Inadequate wedging of pipes was provided. Proposed Improvement Measures.

The following proposed measures listed below should be identified, implemented, monitored, evaluated and controlled. 4.1 4.2 4.3 4.4 4.5 Maximum of three layers of pipe pile casings stacking were recommended. Adequate precast concrete block wedging must be provided for stacking of pipes. The pipes should be securely fixed by U-clamp to each other of pipes as shown in Photo 2. A safe system of work should be developed and implemented in connection with the stacking of pipe pile casings (610X28mm, 9M). Provision of appropriate and sufficient safety training, information, supervision and instruction to the crane operator, signalman and rigger before stacking and lifting operation of pipes.

Incident Report on Collapse of Stacking of Pipe Pile Casings at WA2 on 25 May 2012 4.6 The work should be supervised by an appointed person who possesses adequate knowledge, substantial experience and appropriate training. 4.7 The working area shall be fenced off to protect the nearby workers. 4.8 Sufficient information and clear instruction on the work procedures should be given. 4.9 Good housekeeping should be maintained within the workplace. 4.10 Regular inspection of workplace should be provided to ensure safe workplace on site. 4.11 More clear sign and notice should be posted at the workplace. 4.12 Risk assessment shall be conducted and safe working procedures shall be developed.
