Facilitator: Pamela May Utoalih Assistant Facilitators: Monica Arvisu Rose Mia Trinidad Participants: Barcelon, Rommel Candari, Benigno Lalas, Jerry Manalansan, Jerry Miranda, Larry Pangilinan, Ramon Objectives: 1. Help patients express their feelings with the use of new words. 2. Reinstate good values to patients with the use of word/s pertaining to a good value. Mechanics: 1. The facilitator will provide a flashcard with the word of the week. 2. The facilitator will then ask the patients to state what the word means to them. 3. Then, the facilitator will give the word-meaning and give sample sentences using the word. 4. The facilitator will ask the patients to use the word in a sentence and give sentence. 5. After the activity, the facilitator will encourage the patients to verbalize their thoughts and what they have learned about the activity. Evaluation: Patients were able to understand the word-meaning and used it to express their emotion. They used the word helpfulness in a sentence or matulungin in Tagalog. They even identified certain people in the community which best describes the word, like a Fireman, Policeman etc. They also stated examples in which they would be helpful or have been helpful inside the ward and outside the institution. The facilitators have not been able to take pictures of the said activity but nonetheless I would say this is a successful activity. Patients participated all throughout and they somewhat directed the flow of the activity.

MUSIC and ART THERAPHY Date: March 6, 2013 Time: 10:00 A.M. To encourage the self-expression of thoughts and feelings. 2. Encourage the development of healthy strategies for coping with hospitalization. 3. Facilitate positive self- esteem and positive body image. Mechanics: 1. The patients will be given a bond paper and coloring materials. 2. They will listen to the music (one is a fast pace song and the other one is in slow pace). 3. Then the patients will draw what theyre thinking or what they are feeling when they hear the music. 4. Lastly, the patients will then describe or explain their drawings. Evaluation: In this activity the patients drew the things that they were thinking at the moment and also the things that they miss outside the hospital. The patients drew different scenarios wherein they are with their families and they also those things that they want to do while they were still outside the hospital like playing Basketball. The patients felt happy when they described what they have drawn in the paper. They are willing to explain what they were thinking of when they were drawing. My suggestion is that a good sound system will make this activity more successful.

PLAY THERAPY: Pinoy Henyo Date: March 8, 2013 Time: 09:30 A.M. The team will be given 2 minutes to get the correct word. No body language is allowed. The team can be disqualified for this. 3. If Player Two answers with another word other than Yes, No or Can Be, a 3-seconds penalty shall be given to the team. The clock stops when Player One answers correctly the secret word. EvaLuation: The Activity helped the patients improve their communication skills by interacting with their team mate. The patient learned that in order for them to guest the word they must have trust with their team mates. Their vocabulary also improved. PatientS had a great time in guessing what the words were. If they were unable to guest a word they would ask for a second try. They would cheer for their co-patient to uplift their spirits.

Certain side-effects of the patients medication (pill-rolling, hand tremors etc.) can be alleviated with the constant rolling of the magazine strips. Mechanics: 1. The facilitator will explain the steps in bead-making. 2. After which, the facilitator will then guide the patients in to the making of the beads. 3. Each patient will be asked to set aside their finished product for quality testing. 4. After the activity, the facilitator will encourage the patients to verbalize their feelings and what they have learned from the said activity. Evaluation: Almost all of the patients were able to follow the instructions well. They showed their talent in arts and crafts. Though all of them showed interest in the activity, few of them were not able to make good beads but were equally regarded as the others. The most number of beads a patient did was 19 for the 30 minute duration of the activity. Some suggested that we do the activity again after lunch. All in all, I can say that this was a successful activity. Not only that they enjoyed the activity, they can also make useof this as a livelihood when they are already discharged from the institution. Documentation:
