Employee Career Development and Performance Evaluation Form


To be completed and provided to supervisor in advance of the career development and performance evaluation meeting. Since this form is the basis of the career development performance evaluation process, please be as detailed, thorough, and thoughtful as possible.

Instructions:
Each LBC employee and his/her supervisor will conduct and complete an annual career development and performance review no later than April 6th 2012. The recommended form for that purpose is attached. That forms contents may be modified, as managers consider appropriate, provided that the revised form addresses both the employees career development plans, provides the employees with performance evaluation information , and addresses the employees compliance with the LBC Code of Business Conduct and Company policies. The employee will initiate the process by completing the form which will be reviewed by his/her supervisor. The two will then hold a face-to-face meeting arranged by the supervisor to discuss the employees career development for the coming fiscal year and performance to date during the current year. They will then reach agreement on the forms final wording and content and the form will be signed by the employee and supervisor. The employee and supervisor will each retain a copy of the completed form. Failure of the supervisor to complete the process and by that date will be a factor in the supervisors performance evaluation.

Performance:
1. Core Duties---Discuss your performance within the framework of the responsibilities of

your position and your compliance with our Code of Conduct and Company policies.
a) On what dimensions of your job do you think you are doing exceptionally well? Letter drafting, immediate dispatch, distributing letter and document, record keeping of letter and other documents, maintaining files, emailing as required. Coordinating with staff. Formatting and preparing project documents such as Pre-Qualification, Bid Documents, Design reports etc.

Describe your greatest accomplishments over the previous time period (Including project performance, personal development, awards, honors, earned certifications, etc.) Working with MS Word, Excel, Power Point, Adobe Photoshop, Adobe PageMaker. Coordinating with Local and Foreign staffs. Especially preparing bid documents and Agreements for all contract packages. c) What elements of your job responsibilities are areas for improvement and what recommendations do you have to improve performance? Understanding each commands of supervisors and engineers and quick responds.

a) Please describe the elements of your job that you enjoy most. I am satisfied to working with this big organization. Working with different persons with new ideas and thoughts in different type environment and working independently in my selected area. b) Please describe the elements of your work that are most frustrating or most harmful to your ability to perform at your best. What recommendations do you have for improvement? Daily routine workload requires some assistance from office boy to avoid pending of works.

Specific training required to the same field where I am working from the beginning of the project. Some systems I should follow which H.O. is following. And some encouragement from the supervisor to do something best. b) Propose for discussion with your supervisor a concrete, actionable plan to accomplish these goals. Highlight areas where the goal requires manager support, additional training, etc. Assigning an assistance (Office Assistant) to manage daily workload.
