STUDENT HEALTH FOLLOW-UP REQUIREMENTS Due to A-SMYLE Program Office: June 1, 2013

Prepared for: Mekic, Senka AIS ID: 2013-031-033-0447 Program Country: SR-99 (Serbia) April 10, 2013 Dear Student, We have reviewed the health information that you and your physician provided to the A-SMYLE Program. The items marked YES on the list below should be obtained BEFORE you travel to the U.S. You must obtain written documentation from your physician that confirms that these requirements were fulfilled and the date. Documentation should be on official letterhead or an official vaccination card and contain the stamp or signature of the doctor or clinic. You must provide copies of these documents to your A-SMYLE program office as soon as possible, but no later than June 1, 2013. You also must hand carry those documents with you to the U.S. Some U.S. schools may require additional vaccinations. If this is the case, you will receive a separate request for follow-up. A. Immunizations
The health certificate that you provided with your scholarship application might indicate that your immunization record is incomplete according to U.S school standards. If so, you need to obtain the following immunizations or booster shots as soon as possible in order to enter school in the U.S. Please have your physician give you the immunizations or boosters listed below that are marked YES and provide you with written evidence that includes all dates of those immunizations, and hand carry paper copy of this documentation with you to the U.S.: Diphtheria, Pertussis, and Tetanus Vaccines: - dose 1 - dose 2 - dose 3 - dose 4

Booster for Diphtheria, Pertussis, and Tetanus Vaccines: Booster: Your record indicates that your last immunization against Diphtheria, Pertussis, and Tetanus was more than nine (9) years ago. You must have had a booster dose of Diphtheria, Pertussis, and Tetanus vaccine within the past nine (9) years. The booster dose must contain pertussis to be accepted.

Poliomyelitis (trivalent oral): - dose 1 - dose 5 Measles, Mumps, and Rubella (MMR): - dose 1 - dose 2 - booster - dose 2 - dose 6 - dose 3 - dose 7 - dose 4 - dose 8

B. Tuberculosis Testing
The health certificate that you provided with your scholarship application might indicate that your evaluation for Pulmonary Tuberculosis (TB) is incomplete. If so, you need to obtain the additional tests below that are marked YES. Please have your physician provide you with written documentation of the results that includes the date and results of the tests, and hand carry a paper copy of this documentation with you the U.S.: Yes TB Skin Test or Blood Test - The health certificate that you provided with your scholarship application indicates your skin test for Tuberculosis was outdated or not provided Therefore, you need to obtain a TB skin test or blood test. Skin test results must be given as a number, in millimeters (mm). Positive and Negative are not acceptable test results. TB Chest X-Ray - Your skin test or other indicators for Tuberculosis were reported to be abnormal. Therefore, you need to obtain a chest X-ray conducted three (3) or more months after the date of the skin test.

A-SMYLE Staff Instructions When you receive updated health documents from students, review the documentation to make sure that the student has provided the information requested in an official format. If the information is not in English, provide a translation. A scan of the original document and the translation should be sent to Skye Wallace Henry at swhenry@americancouncils.org. The information requested may be recorded on the Health Certificate Addendum.
